Özet

1986-2010 yılları arasında Pediatrik Oftalomoloji biriminde, doğumsal veya gelişimsel katarakt tanısıyla lens aspirasyonu yapılarak takip edilen olguların görsel sonuçları ile görme prognozunu etkileyen faktörlerin incelenmesi hedeflendi. En az 12 ay takip edilen 54 olgunun 85 gözü araştırmaya alındı. Operasyon sonrası ortalama takip süresi, 95±7.8 ay olan olguların cerrahi öncesi en iyi düzeltilmiş görme keskinliği (EİDGK) 0.03’ten, cerrahi sonrası anlamlı şekilde artarak 0.32’ye çıktı. Operasyon öncesi nistagmusu veya kayması olan olguların, operasyon sonrası EİDGK’leri anlamlı düzeyde daha düşük bulundu (sırasıyla p=0.001, p=0.002). Nistagmusu olan hiçbir olguda operasyon sonrası EİDGK 0.5’e ulaşmadı. Operasyon öncesi kayma tespit edilen 27 gözün %29.6’sında, kayması olmayan 54 gözün ise %61.1’inde, EİDGK 0.4 ve üzerinde ölçüldü. 55 gelişimsel kataraktın %94.5’inin, 26 doğumsal kataraktın ise %30.8’inin operasyon sonrası EİDGK’si 0.1 ve üzerinde ölçüldü. Pediatrik kataraktın tek taraflı ve doğumsal oluşunun, operasyon öncesi kayma, nistagmus ve oküler patoloji varlığının, görsel prognozu kötü etkilediği saptandı.

Abstract

The purpose of this study was to evaluate the visual results and the factors which effects the visual prognosis after lens aspiration of the cases with the diagnosis of congenital or developmental cataracts in Ophthalmolgy Clinics between 1986-2010. 85 eyes of 50 cases with at least 12 months follow up period, were included to the study. Mean follow up time was 95±7.8 months. Post-operative best corrected visual acuity increased from 0.03 to 0.32 significantly. Best corrected visual acuity of the cases with nystagmus or strabismus were significantly low (p=0.001, p=0.002, respectively). Best corrected visual acuity after operation was not better than 0.5 in the cases with nystagmus. Post-operative best corrected visual acuity was 0.4 or better in 29.6 %of 27 eyes with tropia and in 61.1%of 54 eyes without tropia which had been detected before the operation. Post-operative best corrected visual acuity was 0.1 or better in 94.5%of 55 eyes with developmental cataracts and in 30.8%of 26 eyes with congenital cataracts. Unilateral, congenital pediatric cataracts; the ones with strabismus and nystagmus have bad visual prognosis.
