On yıllık perkütan nefrolitotomi deneyimlerimiz: retrospektif çalışma

Özet
Amaç: Kliniğimizde taş cerrahisinde oldukça sık olarak kullanılan bir yöntem olan perkütan nefrolitotominin (PNL) 976 hasta üzerinden değerlendirilmesi Gereç ve Yöntem: Temmuz 2003 ile Mart 2013 tarihleri arasında kliniğimizde 976 hastaya (568 erkek/408 kadın) perkütan nefrolitotomi operasyonu uygulandı. Perkütan gi- rişim C kollu floroskopi altında hastaya yüzüstü (prone) po- zisyonu verilerek yapıldı. Amplatz dilatatörlerle giriş yolu genişletildi ve 30 F Amplatz sheat yerleştirildi. Perkütan taş kırma ve taşların temizlenmesi 26 F rigid nefroskop ve pnömatik litotriptörle yapıldı. Operasyonun bitiminde 18 F nefrostomi tüpü takıldı. Dokuz yüz yetmiş altı olgunun so- nuçları karşılaştırıldı. Bulgular: Yaş ortalaması 47,6±10 (17-76) olan 568 erkek, 408 kadın toplam 976 olguya PNL uygulandı. Beş yüz otuz altı sağ, 440 sol üniteye PNL operasyonu yapıldı. Hasta- ların taş yükü 8,1±4,21 cm2 (2-30 cm2) bulundu. Ortalama operasyon süresi 72,1±29,7 dk.’ydı (30-80 dk.). Taşsızlık oranı 849/976 (% 87) olarak hesaplandı. Postoperatif dö- nemde 46 (% 4,8) hastaya kan transfüzyonu yapıldı. On altı hastada komplike İYE gelişti. Nefrostomi kalış süresi orta- lama 2,6 gün, hastanede kalış süresi ortalama 3.4 gündü. Sonuç: Perkütan nefrolitotomi geniş serili çalışmamızda görüldüğü üzere etkinliği yüksek güvenle uygulanabilecek bir yöntemdir.
Abstract
Objective: The evaluation of the commonly used procedure Percutaneous Nephrolithotomy (PNL) on 976 patients with stone disease. Material and Methods: 976 patients (568 men/408 women) underwent percutaneous nephrolithotomy operation betwe- en July 2003 and May 2013. Giving the patient prone posi- tion percutaneous intervention was performed under C-arm fluoroscopy. 30 F Amplatz sheath was placed after enlar- gement of the entrance way with Amplatz dilatation. 26 F rigid nephroscope and pneumatic lithotriptor were used to disintegrate and cleane the stones. 18 F nephrostomy tube was placed at the end of the operation. The results of 976 cases were compared. Results: The mean age of 47,6±10 (17-76) of 568 men and 408 women totally 976 patients were performed PNL. PNL was applied to 536 right, 440 left unit. The stone burden was found to be 8,1±4,21 cm2 (2-30 cm2). The average operation duration was 72,1±29,7 minutes (30-80 minu- tes). The stone free rate was calculated as 849/976 (87 %). 46 patients (4,8 %) were applied blood transfusion during postoperative period. Complicated urinary tract infection developed in 16 patients. The average duration of stay for nephrostomy was 2,6 days and the average lenght of hospi- tal stay was 3,4 days. Conclusion: As we see in our study with large series, per- cutaneous nephrolithotomy is a highly effective and safe method in stone diseases.
