İntramedüller yerleşimli glioblastom olgusuna yaklaşım: Bir olgu sunumu

Özet
Glioblastom (GB) yetişkinlerde beyinde görülen en sık glial tümördür. Spinal kökenli GB olguları ise ol- dukça ender görülmektedir. Bulgular: 41 yaşındaki erkek hasta Ekim / 2009da Torakal 6-8 seviyesi aralığında spinal kitle tespit edilerek opere edilmiş. Patolojisinde, nekroz ve inf- lamasyon çevresinde gliozis alanları saptanmış ve takip edilmiştir. Şubat / 2011de yakınma progres- yonu ve nüks tespit edilen hastaya ikinci operasyon uygulanmıştır. Patolojik incelemesinde dev hüc- reli GB tanısı almıştır. Subtotal cerrahi rezeksiyon sonrası tümör lojuna radyoterapi (RT) ve eşzamanlı Temozolamid tedavisi uygulanmıştır. Adjuvan te- davisi tamamlanan hastanın ilk takibinde yaygın leptomeningeal tutulum ve akut triventriküler hidrosefali saptanmıştır. Ventriküloperitoneal şant takılarak semptomatik destek tedavileri ile izlenen hasta 3 ay sonra hastalığın ilerlemesi sonucunda kaybedilmiştir. Sonuç: Spinal GB olguları son derece ender görülen ve agresiv seyir gösteren tümörlerdir. Bu olgular- da leptomeningeal yayılım ve intrakranial metas- taz riski yüksektir. Uygun olgularda postoperatif kranio-spinal RT uygulaması, hastalık kontrolü açı- sından etkin bir tedavi yaklaşımı olabilir.
Abstract
Background: Glioblastoma (GB) is the most com- mon glial cell tumor of the adult brain. However, primary GB of the spinal cord is a rare condition. Case Description: A 41-year-old male patient was operated for the T6-T8 intramedullary lesion in Oc- tober/2009, and histopathology turned out to be peripheral gliosis around necrosis and inflamation areas. When his symptoms are progressed; subtotal resection was performed for recurrent spinal lesion in February/2011 and it was reported as spinal gi- ant cell GB. After subtotal resection; external beam radiotherapy and concomitant temozolamid were performed. Leptomeningeal dissemination to the brain and acute ventricular hydrocephalia were de- tected in the first follow up control after comple- tion of chemoradition. Ventriculoperitoneal shunt was placed and the patient was followed up with supportive care and subsequently died due to dise- ase progression three months later. Conclusion: Spinal GB is an extremely rare and agg- ressive disease. Leptomeningeal dissemination and intracranial metastases is frequently observed in spinal GB. When amenable postoperative cranios- pinal RT can be an efficient treatment method for disease control.
