Özet

Amaç: Bu çalışmada renal hücreli kanserde multifokalite oranı ve renal hücreli kanser için prognostik faktörlerin klinik, anatomik ve histolojik özelliklerinin multifokalite ile olan ilişkileri incelenerek, endikasyonları genişlemekte olan nefron koruyucu cerrahiye uygun hasta seçiminde öne çıkan sonuçlar irdelenmiştir. Yöntemler: Bu çalışmaya Mayıs 1996 - Şubat 2010 tarihleri arasında İstanbul Okmeydanı Eğitim ve Araştırma Hastanesi 2. Üroloji Kliniği’nde böbrek tümörü tanısıyla radikal nefrektomi uygulanan ve patoloji sonucu renal hücreli karsinom (RHK) olarak rapor edilen 294 hasta dahil edildi. Böbrek tümörü tanısıyla nefron koruyucu cerrahi uygulanmış veya RHK dışında rapor edilmiş patolojiye sahip olan hastalar çalışma dışında bırakıldı. Çalışmaya dahil edilen 294 hastanın ameliyat öncesi bulguları ve ameliyat sonrası patolojileri incelendi. Bulgular: Çalışmaya alınan hastaların 209’u (%71.1) erkek, 85’i (%28.9) kadındı. Tümör en büyük çapları ortalama 8,4±4.2 cm (1-28 cm) olarak tespit edildi. Tümörlerin böbrekteki lokalizasyonları 108 hastada (%37) üst 1/3, 91 hastada (%31) orta 1/3 ve 95 hastada (%32) alt 1/3 olarak tespit edildi. RHK nedeniyle radikal nefrektomi uygulanan 294 hastanın 14’ünde (%4.76) patolojik olarak multifokalite saptandı. RHK’de multifokalitenin cinsiyet, tümörün sağ veya sol böbrek kaynaklı olması, tümörün böbrekteki lokalizasyonu, tümör çapı, patolojik evre, kapsül invazyonu, lenf nodu tutulumu, metastaz varlığı, Fuhrman nükleer derecelendirmesi, tümör histolojik tipleriyle istatistiksel anlamlı ilişkisi saptanmadı. Sonuç: Böbreğin başka bir bölgesinden kaynaklanan nüksün oranını saptamak amacıyla araştırılan multifokalitenin bu çalışmada incelenen prognostik faktörlerle korelasyonu istatistiksel olarak anlamlı bulunmamıştır. NKC (nefron koruyucu cerrahi) medikal ve cerrahi olarak uygun görülen tüm RHK’lerde güvenli bir cerrahi teknik olduğunu düşünmekteyiz.

Abstract

Objective: This study focuses on patient selection for nephron-sparing surgery, indications of which have extended, by analyzing the multifocality rate of renal cell carcinoma and its relationship with clinical, anatomical and histological properties of prognostic factors. Methods: 294 patients admitted to Istanbul Okmeydanı Training and Research Hospital 2 nd Urology Clinics between May 1996 and February 2010 with a history of radical nephrectomy for renal tumor whose histopathology revealed renal cell carcinoma (RCC) were included in the study. Patients who had undergone nephron-sparing surgery for renal tumor and whose pathology did not reveal RCC were excluded. Preoperative findings and postoperative pathology reports of 294 patients were examined. Results: Of the patients included in the study, 209 (71.1 %) were male and 85 (28.9 %) were female. The mean largest diameter of the renal tumors was 8, 4±4, 2 cm (1-28 cm). Renal cell tumors were localized in the upper (n=108 patients: 1/3) mid (n=91: 31 %) and lower (n= 95: 32 %) segments of the kidneys. Out of 294 patients who had undergone radical nephrectomy for RCC, histopathology revealed multifocality in 14 (4.76 %) patients. Statistically significant correlation was not observed between multifocality of RCC and sex, laterality, and location of the tumor, tumor diameter, pathologic stage, capsular invasion, lymph node involvement, presence of metastasis, Fuhrman nuclear grading and histologic types of the tumor. Conclusion: The correlation of multifocality which was searched for to detect recurrence rate of the tumor from another part of the kidney, with prognostic factors analyzed in the study was not statistically significant. We think that nephron-sparing surgery is safe for all RCCs with suitable medical and surgical indications.
