Özet

Amaç: Bu çalışmadaki amacımız; cerrahi insizyon ile başlayan nöropatik ve inflamatuvar ağrıda, pe- riferik ve santral antinosiseptif etkinliği gösterilen gabapentinin postoperatif tramadol tüketimi, eks- tübasyon süresi, vizüel ağrı skorlarına etkisini ve oluşabilecek yan etkilerini; gabapentinin tek başı- na kullanımı ve plasebo grubu ile karşılaştırmaktır. Gereç ve Yöntemler: Supratentorial tümör rezeksi- yonu planlanan 40 hasta rastgele iki gruba ayrıldı. Operasyondan 1 saat önce; Grup K’ye 1 adet pla- sebo kapsül, grup G’ye 600 mg gabapentin tablet oral olarak verildi. Tüm olgulara standart aneste- zi verildi. Peroperatif 30 dk. aralıklar ile OAB’ları, KAH’ları ve remifentanil tüketim miktarları kayde- dildi. Olgulara, operasyon bitimine doğru 1 mg/kg hasta kontrollü analjezinin yükleme dozu yapıldı ve sözlü uyaranlara yanıt alındıktan sonra derlen- me odasına alındı. Tüm olgulara HKA ile tramadol uygulandı (yükleme dozu 25 mg, kilitli kalma süresi 30 dk.). Operasyondan sonraki 1., 2., 3., 6., 12. ve 24. saatlerde, vizüel ağrı skoru, tramadol tüketimi ve yan etkiler takip edilip, kaydedildi. Bulgular: Postoperatif dönemdeki toplam trama- dol tüketimi; Grup K’de 271.0±59.8 mg, grup G’de 218.7±71.1 mg olarak bulundu. Grup K’deki toplam tramadol tüketimi grup G’ye göre anlamlı yüksekti. Gabapentin grubundaki VAS değerleri plasebo gru- buyla karşılaştırıldığında tüm ölçüm zamanlarında dikkate değer bir şekilde düşüktü. Grup G’nin top- lam remifentanil tüketimi Grup K’ya göre anlamlı olarak düşük bulundu. Postoperatif bulantı-kusma grup K ile karşılaştırıldığında grup G’de daha az sıklıkta gözlendi. Sonuç: Supratentorial tümör cerrahisi geçirecek hastalarda gabapentin uygulanması postoperatif dönemde etkin bir analjezi sağladığı gibi bu dö- nemdeki analjezik gereksinimini de azaltmaktadır.

Abstract

Objective: The aim of this study was to compare the effects of gabapentin, which has peripheral and central antinociceptive action in neuropathic and inflammatory pain that starts with surgical incision, to that of placebo regarding the effects on trama- dol consumption, tracheal extubation time, visual pain score and adverse events. Material and Methods: Forty patients undergoing craniotomy for supratentorial tumor resection were randomly assigned into two groups. In group K (n=20) one placebo capsule and Group G 600 mg gabapentin (n=20) were administered one hour before surgery. All patients received standard anesthesia regimens. Heart rate (HR), mean arterial pressure (MAP), total remifentanil consumption of the patients were eva- luated and recorded per 30 minutes in perioperative period. Before the end of operation 1 mg/kg PCA was started with loading dose and patients were taken to recovery room after obtaining response to verbal sti- mulus. All patients received patient-controlled anal- gesia with tramadol with a 25 mg initial loading dose and 30- min lockout interval. Tramadol consumption, VAS score and side effects were recorded at 1., 2., 3., 6., 12. and 24. hours after the operation. Results: Postoperative total tramadol consumption was 271.0±59.8 mg in group K and 218.7±71.1 mg in Group G. Tramadol consumption in group K was sig- nificantly higher than Group G. Patients in the ga- bapentin group had significantly lower pain scores at all time intervals in comparison to placebo group. Intraoperative total remifentanil consumption was significantly lower in group G than in Group K. The incidence of postoperative nausea and vomiting was less frequent in group G compared with group K. Conclusion: The administration of gabapentin to patients undergoing supratentorial tumor resection was effective for acute postoperative pain. It also decreased analgesic consumption after surgery.
