Koksartroz tanılı olgularda floroskopi altında uyguladığımız intraartiküler kortikosteroid enjeksiyonunun üç aylık verileri

Özet
Amaç: İntraartiküler kortikosteroid enjeksiyonları (İKE), osteoartritik eklemlerin konservatif tedavisinde yer alan bir tedavi seçeneğidir. Diz eklemi için pek çok çalışma bulun- masına reğmen, kalça osteoartritinde etkisini değerlendiren çalışmaların sayısı nispeten azdır. Bu çalışmada koksartroz olgularında İKEnin semptomlar ve yaşam kalitesine etki- sinin araştırılması amaçlanmıştır. Gereç ve Yöntemler: Otuz bir koksartroz tanısı olan has- taya 7 mg betametazon floroskopi altında kontrast madde eşliğinde intraartiküler enjeksiyon şeklinde uygulandı. Ağrı şiddeti vizuel analog skala ile (VAS-gece, VAS-hareket, VAS-istirahat), fonksiyonel düzeyleri Western Ontario ve McMaster Universitesi Osteoartrit indeksi (WOMAC) ve yaşam kalitesi ise SF-36 ile enjeksiyon öncesi, enjeksiyon sonrası 24. saat, 1. hafta, 1. ay ve 3. ayda değerlendiril- miştir. Bulgular: Grubun tamamında enjeksiyon sonrası ilk 24 sa- atte ve 1. haftada değerlendirilen VAS-gece, VAS-istirahat ağrısı ve VAS-hareket düzeylerinde istatistiksel anlam- lı iyileşme mevcuttu (p<0,01). 1. (p<0,05) ve 3. aylarda (p<0,05) ise tedavi öncesine göre hastaların tamamında, sadece VAS-gece skorunda istatistiksel anlamlı düzelme vardı. Western Ontario ve McMaster Üniversiteleri Osteo- artrit İndeksi (WOMAC) total skorları ve SF-36 ile değer- lendirilen yaşam kalitesinde ise tedavi öncesine göre 24. saat, 1.hafta, 1. ay ve 3. ayda istatistiksel olarak anlamlı iyileşme mevcuttu (p<0,05). Sonuç: Kliniğimizde uygulanan intraartiküler kortikoste- roid enjeksiyonu hastaların klinik parametrelerinde, fonksi- yonel düzeylerinde ve yaşam kalitelerinde ilk 3 ay boyunca anlamlı düzelme sağlamıştır.
Abstract
Objective: Intra-articular corticosteroid injection (ICA), is a treatment option used in the conservative treatment of osteoarthritic joints. Although there has been a lot of work for the knee joint, number of studies evaluating the effect of ICA on hip osteoarthritis was relatively small. In this study, we aimed to investigate the effect of ICA on symptoms and on quality of life in patients with coxarthrosis. Material and Methods: 31 patients diagnosed with coxart- hrosis included in our study and 7 mg betamethasone in- jection was applied under fluoroscopy. The severity of pain was evaluated by visual analog scale (VAS-night pain, VAS- activity, VAS-rest), functional levels by Western Ontario and McMaster University Osteoarthritis Index (WOMAC) and quality of life by the SF-36 in period of pre-injection, post-injection 24 hours, 1 week 1 months and 3 months. Results: Significant improvements were present in VAS- rest, VAS-night pain, VAS-activity during the first 24 hours and 1. week after injection (p<0.01) and also significant improvement was found in VAS-night pain during 1. month (p<0.05) and 3. months (p<0.05) compared to the pre- treatment measurements in all patients. There was a sta- tistically significant improvement in Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) to- tal scores and the quality of life as assessed by the SF-36 compared to baseline during 24 hours, 1 week, 1 months and 3 months post-injection (p<0.05). Conclusion: In our clinic, patients who underwent intra- articular corticosteroid injection showed significant imp- rovement during the first 3 months in terms of of clinical parameters, functional status and quality of life.
