Özet
Amaç: Popliteal arter anevrizmaları nadir görülme- sine rağmen ekstremite kaybıyla sonuçlanan ciddi komplikasyonlara da sebep olmaktadır. Popliteal arter anevrizmalarının cerrahi tedavisinde; birinci hedef tromboembolizm ve onun neden olabileceği ekstremite iskemisi ve ekstremite kaybını ortadan kaldırmak, ikinci hedef ise anevrizmanın genişle- mesini ve rüptürünü önlemektir. Bu çalışmamızda ki amacımız popliteal arter anevrizması tanısıyla cerrahi müdahale yapılan olguların postoperatif sonuçlarını sunmaktır. Gereç ve Yöntem: Ocak 2000 - Ocak 2012 tarihleri arasında Bezmi Alem Vakıf Üniversitesi Tıp Fakülte- si Kalp ve Damar Cerrahisi kliniğinde popliteal ar- ter anevrizması tanısıyla gelen 16 hasta çalışmaya alındı. 3 olguda acil, 13 olguda ise elektif cerrahi onarım yapıldı. Bulgular: Acil cerrahi girişim yapılan üç olgunun bi- rinde akut ekstremite iskemisi düzelmedi ve sol alt ekstremite diz altı amputasyonu yapıldı. 2 olguda yara yeri enfeksiyonu görüldü, verilen yoğun me- dikal tedavi ile gerilediği görüldü. Bir olguda greft tıkanıklığı nedeniyle greft çıkarıldı. Sonuç: Popliteal arter anevrizmaları klinik olarak genellikle asemptomatik seyretmesine rağmen, ekstremite kaybıyla sonuçlanan ciddi komplikas- yonlar da bulunmaktadır. Bu nedenle zamanında yapılan müdahale hayat kurtarıcı olmaktadır.
Abstract
Objective: Although rarely encountered, the popli- teal artery aneurysms may cause serious complicati- ons resulting in loss of limb. The first goal of in the surgical treatment of popliteal artery aneurysms is to prevent venous thromboembolism and to elimi- nate limb ischemia and limb loss that could deve- lop consequently; the second goal is to prevent the expansion of aneurysm and rupture. The aim of this study, is to present the postoperative results of patients who underwent surgical intervention with the diagnosis of popliteal artery aneurysms. Material and Methods: 16 patients who were ad- mitted to the Bezmi Alem Vakıf University School of Medicine and Cardiovascular Surgery between January 2000 - January 2012 with the diagnosis of aneurysm of the popliteal artery were included in the study. In three cases emergent and in 13 cases elective surgical repair was performed. Results: In one of the patients who underwent emergent surgical intervention acute limb ische- mia did not improve and the patient underwent below-knee amputation of the left lower extre- mity. Wound infection was seen in 2 patients and improvement was detected with the intensive me- dical treatment. In one case, the graft was removed because of graft occlusion. Conclusion: Although the popliteal artery ane- urysms had usually asymptomatic clinical course, serious complications resulting in loss of limb can be seen. So early intervention might be life saving.
