Septik Artrit Tablosu ile Başvuran Akut Lenfoblastik Lösemi Olgusu

Özet

Çocukluk çağında akut lösemi sıklıkla kas iskelet sistemini tutar ve başlangıçta değişik ortopedik durumları taklit ede-bilir. Öykü ve klinik bulgular yanıltıcı olabilir ve hematolo-jik ve radyolojik değerlendirmeler tanı koymada yardımcı olmayabilir. Üç yaşında erkek hasta sağ dirsekde şişlik ve ağrı nede-niyle başvurdu. Septik artrit tanısıyla altı hafta antibiyotik tedavisi aldı. Fizik muayene ve laboratuvar bulguları nor-male döndü. Çekilen kontrol sağ kol grafisinde metafizde osteoporoz ve periost reaksiyonu saptandı. Yapılan kemik iliği incelemesi sonucunda akut lösemi tanısı konuldu. Olgumuz ortopedik sorunlar ile başvuran çocukların ayırı-cı tanısında akut löseminin düşünülmesi gerektiğini vurgu-lamak için sunulmuştur.

Abstract

In childhood, acute leukemia commonly involves the muscu-loskeletal system and it can mimic orthopedic pathologies at the beginning of the disease. History and physical examina-tion findings may be misleading. Hematological and radio-logical evaluations may not be helpful in the diagnosis. Our case is a 3 years old boy applied to us with swelling and pain in his right elbow. He was diagnosed as elbow septic arthritis and treated as septic arthritis for 6 weeks. After the end of treatment, patient's physical examination and laboratory findings were normal, but control x-ray pre-sented abnormal periosteal reaction and osteoporosis. He was searched again and diagnosed as acute leukemia by bone marrow evaluation.The case was presented to express once again; that acute leukemia must be kept in mind in the differential diagnosis of children with orthopedic problems.
