Sakrospinöz Ligament Fiksasyon Olgularının Analizi ve İntraoperatif Kanama Komplikasyonunun Literatür ile Birlikte İrdelenmesi

Özet
Amaç: Kliniğimizde hem vajinal kaf hem de uterin prolapsus tedavisi için sakrospinöz ligament fiksasyonunu yapılan 22 hastada operatif bulgular ve intraoperatif komplikasyonları analiz etmek.Gereç ve Yöntemler: Bu çalışma İstanbul Eğitim ve Araştırma Hastanesi Genel Jinekoloji Kliniğinde retrospektif olarak yürütüldü. Çalışmaya 2008-2014 yılları arasında 3 ayrı jinekolojist tarafından sakrospinöz fiksasyon yapılan 22 olgu dâhil edildi. Preoperatif öykü, demografik bilgiler, uterus ve vajinal kaf prolapsusu için Baden Walker klasifikasyonu, operatif ve postoperatif bulgular ve komplikasyonlara hasta dosyalarından ulaşıldı.Bulgular: Hastaların yaş ortalaması 59,09 (minimum: 40, maksimum: 81). Median gravida 3 ve parite 2 . Sakrospinöz ligament fiksasyonu için endikasyon 12 olguda uterin prolapsus, 10 olguda vajinal kaf prolapsusu idi. Vajinal histerektomi tüm uterin prolapsus olgularına uygulandı. Ek cerrahi işlem (kolporafi anterior, posterior, enterosel) 17 hastaya yapıldı. Bir intraoperatif kanama sütürasyon sırasında izlendi. Kanama hemoklip ile durduruldu. İntraoperatif bağırsak ve mesane yaralanması izlenmedi. Postoperatif komplikasyon yoktu.Sonuç: Sakrospinöz ligament fiksasyonu pelvik organ prolapsusunda güvenli ve kullanılabilir bir yöntemdir. Sütürün güvenli bir şekilde atılması yeterli tecrübe ve uygun bir öğrenme eğrisi gerektirir.
Abstract
Objective: The aim of the study was to analyse clinical and operative findings and intraoperative complications in 22 of the patients who were performed sacrospinous ligament fixation for treatment of both vaginal cuff and uterine prolapses in our clinic.Material and Methods: This is a retrospective study conducted at Istanbul Education and Research Hospital, General Gynecology Clinic. Twenty-two patients who had undergone sacrospinous ligament fixation, performed by three different gynecologists between 2008 and 2014 were enrolled in the study. Preoperatively, history, demographical data, Baden Walker Classification for evaluation of uterine and vaginal cuff prolapse, operative and postoperative findings and complications were obtained from patients' database.Results: The mean age of patients was 59.09 (minimum: 40, maximum: 81). The median gravida was 3 and parity was 2 . Indications for sacrospinous ligament fixations were prolapse of uterus in 12 cases and vault prolapse in 10 cases. Vaginal hysterectomy was performed in all cases of uterine prolapses. Concomitant surgery (colporaphy anterior and posterior, enterocele) was performed in 17 patients. One bleeding complication occurred during suturation. The bleeding was controlled by hemoclip application. There were no intraoperative bladder or bowel injuries. There was no postoperative complication.Conclusion: Sacrospinous ligament fixation is a safe and feasible method in patients who had pelvic organ prolapse. Safe insertion of suture requires sufficient experience, and an adequate learning curve.
