Perkutan koroner revaskülarizasyon sonrası minör miyokart hasarı

Özet
Amaç: Başarılı PTKA (Perkutan translüminal koroner anjiyoplasti) yapılan ve/veya stent uygulanan hastalarda minör miyokardiyal hasan (MMH) göstermek, MMH'nın belirlenmesinde troponin T'yi Kreatin kinaz (CK) ve kreatin kinazın MB izoformu (CK-MB) ile karşılaştırmaktır. Gereç ve Yöntem: Çalışma prospektif olarak seçilen, kararlı anginası olan ve tek damara balon anjiyoplasti ve/veya stent uygulanan 100 hastada (69 kadın 31 erkek) hastada yapıldı. Tüm hastalarda işlemden hemen önce ve işlem sonrası 12. ve 24. saatlerde CK, CK-MB ve troponin T değerlerine bakıldı. Bulgular: Hastaların 55'ine PTKA, 45'ine stent uygulandı. PTKA grubunda %13.3, stent grubunda % 20, CK ve CK-MB stent grubunda %16.3, PTKA grubunda %13.3 hastada yüksek saptandı. Her iki grupta da T değerinin artış sıklığı ve derecesi CK-MB'ye göre daha fazla bulundu, fakat bu artış istatistiksel olarak anlamlı değildi. CK ve CK-MB'nin troponin T değeri yükselen 17 hastanın 15'inde yükselmiş olduğu, troponin T değeri yükselen 2 hastada normal sınırlarda kaldığı görüldü. Demografik özellikler, hasta damar sayısı, lezyon tipi, stent uygulama nedeni, stent çapı, stent uzunluğu, balon inflasyon sayısı ve balon inflasyon süresi troponin T değeri yükselen hastalarla negatif olan hastalar arasında benzer bulundu. Troponin T düzeyi yükselen hastalarda hastane dönemi ve 6 aylık takipte yeniden revaskülarizasyon ihtiyacı, akut miyokard infarktüsü ve ölüm gözlenmedi. Sonuç: Her iki uygulamada da MMH oluşabilmektedir. Stent sonrası daha yüksek oranda MMH oluşumuna rağmen, troponin T artışının sıklığı ve derecesi PTKA grubuna göre anlamlı farklılık göstermemektedir. Troponin T düzeyi yükselen hastalarda prognozun etkilenmemesi MMH'm geri dönüşümlü olduğunu düşündürebilir. Troponin T nin CK ve CK-MB'ye göre daha fazla sayıdaki hastada yüksek bulunması, bu proteinin minör miyokardiyal hasarı belirlemede daha duyarlı olduğunu göstermektedir.
Abstract
Aim: is to indicate minor myocardial injury (MMI) in patients who have undergone successful percutaneous transluminal coronary angioplasty (PICA) with or without stent implantation and to compare troponin T with creatine kinase (CK) and its isoform, CK-MB in determination of the MMI. Material and Methods: The study was designed with prospective manner in 100 patients (69 women and 31 men) who had stable angina pectoris and undergone single vessel balloon angioplasty with or without stent implantation. Troponin T, CK, and CK-MB values were measured before and after (12. and 24. hours) the procedure in all patients. Results: Fifty-five patients underwent PTC A and 45 patients underwent stent implantation. Troponin T was found to be increased in 13.3% patients in PTCA group and 20% patients in stent group, CK and CK-MB was found to be increased in 16.3% patients in stent group and 13.3% patients in PTCA group. The frequency and the level of troponin T increase was statistically insignificant. CK and CK-MB values were increased in 15 out of 17 patients whose troponin T levels were high. Demographic data, diseased vessel number, lesion type, indications for stent implantation, stent's diameter, stent's longitude, balloon inflation number and balloon inflation time were found similar between the patients whose troponin T level were high and negative. There were not acute myocardial infarction, death and repeat revascularization in both hospitalization period and sixth month follow-up with patients who had high troponin T level. Conclusion: MMI can occur after both procedures. In spite of higher incidence of MMI after stent implantation, the frequency and the level of troponin T increase were insignificant when compared PTCA. Though the prognosis is not affected among the patients whose troponin T levels are high, we can speculate that MM is reversible. The higher incidence of increased troponin T level after both procedures prove us that this protein is more sensitive than CK and CK-MB in determination of the MMI.
