Diabetes mellituslu hastalarda işitsel beyin sapı cevapları

Özet

Bu çalışmada diabetes mellituslu (DM) hastalarda muayene ile veya klasik odyolojik tetkiklerle tespit edilebilecek seviyede işitme kaybı ortaya çıkmadan önce, işitme yollarında oluşabilecek değişikliklerin işitsel beyin sapı cevaplarına (ABR) etkisi araştırılmıştır. Bu amaçla işitmesi saf ses odyometride normal olan tip-l (n=17) ve tip-ll (n = 26) diabetes mellituslu, 2 hasta grubu ile sağlıklı olan bir kontrol grubu oluşturularak ABR cevapları incelenmiştir. Hasta gruplarında ve kontrol grubunda ABR dalga formunun normal olduğu görülmüştür. Tip-l DM grubunda l. dalga latanslarında, tip-ll diabetes mellitus grubunda ise I, III. ve V. dalga latanslarında kontrol grubuna göre anlamlı gecikmeler tespit edilmiştir (p<.05). Tip-ll DM grubunda ayrıca III-V ve I-V interpik latanslarının kontrol grubuna göre anlamlı derecede uzamış olduğu görülmüştür (p<.05). Sonuç olarak, diabetes mellitusun VIII. sinirde yapabileceği harabiyet klasik yöntemlerle erken dönemde tespit edilemeyebilir. ABR latanslarındaki değişiklikler bu hastalarda, ileride oluşabilecek VIII. sinir hasarı konusunda uyarıcı olmalı ve bu hastalarda hipoglisemi ve ketoasidoz ataklarının önlenmesi için daha dikkatli davranılmalıdır.

Abstract

The aim of this study is to observe the effects of pathologic changes of the auditory, pathways on auditory brainstem response (ABR) in patients with diabetes mellitus (DM) before appearence of hearing loss which can be detected by clinical examination or conventional audiological tests. Two groups of patients with diabetes mellitus, type-l (n=17) and type-ll (n=26), who have normal hearing level on pure tone audiometry and healthy subjects as a conrol group were included in the study and their ABR recordings were evaluated. Wave forms of ABR were normal in patients' group and in control group. Mean latency of the wave-l was significantly longer in the group of DM-I as compared with conrol group (p<.05). Mean latencies of the waves-l, III and V was significantly longer in the group of DM-II as compared with conrol group (p<.05). Interpeak latencies of the III-V and I-V were also found to be significantly longer than in control group (p<.05). In conclusion damages of the vestibulocochlear nerve due to diabetes mellitus can not be determined in the early period by conventional hearing tests. Changes of latencies of ABR in patients with diabetes mellitus, should alert us for a possible damage of the cochleovestibular nerve and a close follow-up is necessary in these patients to prevent hypoglisemia and ketoacidosis attacks.
