Özet

Kliniğimizde Kasım 1990 ve Mart 1993 tarihleri arasında kunt karın travmasına bağlı karın içi kanaması olan 25 olguda; nonoperatif tedavinin yeri, bilgisayarlı tomografi (BT) ve ultrasonografinin (US) tanı ve izlemedeki değeri, prospektif olarak değerlendirildi. Resüsitasyona cevap vermeyen iki olgu (% 8) cerrahi işleme tabi tutulurken, 23 olgu (% 92) nonoperatif olarak takip ve tedavi edildi. Bütün olgulara periton aspirasyonu yapıldı. Nonoperatif olarak takip ve tedavi edilen olguların hiçbirinde erken ya da geç dönemde herhangi bir komplikasyon gözlenmedi. Travmanın akut döneminde BT ile solü organ lezy'onları % 95.6, karın içi serbest sıvı % 100, US ile solit organ lez-yonları % 39.1, karın için serbest sıvı % 78.2 oranlarında gösterildi (p 0.05).

Abstract

Nonoperative management of blunt liver and splenic trauma in children-comparison of ultrasonography with computed tomography During the period of November 1990 to March 1993, the efficacy of computerized tomography (CT) and ultrasono¬graphy (US) was studied in assessing abdominal trauma and monitoring in its non-operative management in 25 children. The study was designed in a prospective manner and covered post-trauma follow-up for three months. All patients underwent abdominal paracentesis, followed by US and CT, if intraperitoneal bleeding was observed. Only 2 of 25 patients (8 %) did not respond to resuscita¬tion and needed surgery. In the initial investigations after trauma CT demonstrated solid organ injury in 95 % of the cases and intraperitoneal fluid (blood) in everycase. US on the other hand showed solid organ injury in 39 % of cases and 78 % of free intraperitoneal fluid (p<0.01, and p<0.05 respectively). Controls with both modalities 3 months after trauma did not indicate a significant dif¬ference between the techniques (p>0.05). The patients tre¬ated surgically or managed non-operatively did not pre¬sent with complications in their long-term follow-up.
