L-arginin ve pentoksifilinin mide iskemi-reperfüzyonu uygulanan sıçanlarda mukoza engeli üzerindeki koruyucu etkileri

Özet
Amaç: Dışarıdan verilen L-Arginin (L-Arg) ve pentoksifiIin (PtX)'in sıçanlarda mideye uygulanan iskemi-reperfüzyon (İR) işleminde mukoza engeli üzerine etkilerini araştırmak. Yöntem: Çalışmada, 28 adet erişkin Wistar Albino cinsi erkek sıçan kullanıldı. Dört kümeye ayrılan sıçanlara ketamin hidroklorid ile anestezi uygulandı. Kontrol kümesine (n=7) sadece laparotomi uygulanırken diğer üç kümedeki sıçanlarda laparotomi sırasında çöliyak artere klemp uygulanarak 30 dk iskemi ve sonrasında 60 dk reperfüzyon uygulandı. İR kümesine (n=7) herhangi bir ilaç verilmedi. Ptx+İR kümesine (n=7) reperfüzyondan önce 50 mg/kg pentoksifilin uygulandı. L-Arg+İR kümesine (n=7) yine repeıfüzyondan hemen önce 3 mg/kg/dk L-Arg verildi. Fizyopatolojik değerlendirmede; mukus miktarı Corne, fosfolipid miktarı Baur ve ülser indeksi makroskobik peteşiyal sayım yöntemi ile belirlendi. Bulgular: İR kümesinde mukus ve fosfolipid miktarı kontrol kümesine göre anlamlı düzeyde daha düşük bulundu (p 0.05). L-Arg+İR Kümesinde fosfolipid miktarı Ptx+IR kümesine göre anlamlı derecede daha yüksek iken (p 0.05). Ülser indeksi, IR kümesinde Ptx+İR ve L-Arg+IR kümelerinden anlamlı olarak daha yüksekti (p<0.05,p<O.OI). Sonuç: Midede İR, hem mukus hem de fosfolipid miktarını önemli ölçüde azaltarak mukoza engelini bozmakta ve ülser indeksini arttırarak mide mukozasında belirgin harabiyet oluşturmaktadır. Dışarıdan verilen L-Arg ve Ptx, bu engel bozukluğunu ve mukoza hasarını azaltmaktadır. Burada L-Arg'in koruyucu etkisi Ptx'kinden daha fazla gibi görünmektedir.
Abstract
Aim: To determine the effects of L-Arginine (L-Arg) and pentoxifylline (Ptx) on mucosal barrier in ischemia-reperfusion (IR) induced gastric injury in rats. Method: Twenty-eight males, adult Wistar Albino rats (200-250 g) were used in the study. The rats were anesthetized by ketamine hydrokloride. Laparotomy only was applied in control group (n=7). In other three groups, coeliac artery was occluded for a period of 30 minutes followed by a repeifusion period of 60 minutes. No drug was given in IR group (n=7). In Ptx+IR group (n=7), 50 mg/kg Ptx was administered intraperitoneally before repeifusion. In L-Arg+IR group (n=7), 3 mg/kg/minute L-Arginine was infused immediately before repeifusion. The amounts of mucus and phospholipid were determined by Corne and Baur's methods. Ulcer index was measured via counting macroscopical petechial hemorrhage. Results: Mucus and phospholipid levels in gastric mucosa were significantly lower in IR group compared to control group (p<0.01 ). The amount of phospholipid was signifi-cantly higher in both Ptx+IR and L-Arg+IR groups when compared to those of IR group (p<0.01 ), while there were no significant differences between Ptx+IR and L-Arg+IR and IR group in terms of mucus amount (p>0.05). Phospholipid level was significantly higher in L-Arg+IR group when compared to Ptx+IR group (p<O.OI), while there was no significant difference between these two groups from the point of mucus amount (p>0.05). Ulcer index was significantly higher in IR group than Ptx+IR and L-Arg+IR groups (p<0.05, p<O.OI). Conclusion: Gastric IR breaks mucosal harrier through significantly decreased mucus and phospholipid levels and results in gastric mucosal damage via increased ulcer index. Ptx and L-Arg diminish barrier breakage and mucosal damage induced by IR. L-Arg seems to protect the gastric mucosal barrier better than Ptx does.
