Üreterosellerin tanı özellikleri ve böbrek-idrar sistemi üzerine etkileri

Özet

Amaç: Bu çalışmanın amacı üreteroselli olgularımızda tanı ölçütlerini ve üreteroselin böbrek-idrar sistemi üzerindeki etkilerini araştırmaktır. Yöntem: 1996-2000 yılları arasında kliniğimizde üreterosel tanısı konan ve tedavi edilen olgular geriye dönük olarak değerlendirildi. Olguların klinik özellikleri, cinsiyetleri, semptomları, tanı yaşları, kullanılan radyolojik tanı [işeme sistoüretrografi (İSÜG), intravenöz ürografi (İVÜ), ultrasonografi (USG) incelemesi] araçları, dimerkaptosüksinik asid (DMSA) böbrek sintigrafileri ve tedavi yöntemleri araştırıldı. Bulgular: Yaşları 1 günlük - 9 yıl arasında olan sekiz olgu değerlendirildi. Altı olgu idrar yolu enfeksiyonu nedeniyle başvurduruldu. Beş olguda İSÜG ile vezikoüreteral reflü (VÜR) saptanırken bunların üçünde üreterosel tanısı kondu. İVÜ yapılan yedi olgunun altısı bu yöntemle üreterosel tanısı aldı. DMSA sintigrafilerinde bir hastada atrofik bir böbrek izlenirken diğer dört hastada nedbeli böbrek görüldü. USG ile sekiz hastanın altısı üreterosel tanısı aldı. Üreterosel tarafında altı hastada değişik derecelerde hidroüreter, hidronefroz, pelvikalisiyel genişleme görülürken; beş olguda ise karşı taraf böbrekte pelvikalisiyel ve üreteral genişlemeler saptandı. Cerrahi tedavide üç hastaya üst pol heminefrektomi uygulanırken ikisinde bu yöntem mesane düzeyi rekonstrüksiyonu ile birleştirildi. İki hastada yalnız üreterosel eksizyonu yapıldı. Üç olguda ise üreterosel eksizyonuna üreteral reimplantasyon eklendi. Üç örneğin histopatolojik değerlendirilmesinde kronik pyelonefrit saptandı. Beş olguda ameliyat sonrası dönemde herhangi bir sorunla karşılaşılmazken, bir olgu farklı nedenlerle kaybedildi ve 2 olgu hipertansiyon ve zaman zaman yineleyen idrar yolu enfeksiyonları nedeniyle izlem altındadır. Sonuç: Sonuç olarak idrar yolu enfeksiyonu özellikle daha büyük yaştaki üreteroselli çocukların tanısında bir anahtardır. Üreterosel tanısı kolay olmakla birlikte tüm böbrek-idrar sistemini tanı anında bile etkilemiş olan karmaşık bir hastalıktır. Olgularımızın özellikleri üreterosellerdeki çok farklı klinik özellikler ve anomaliler nedeniyle tedavide bireyselliği öneren araştırmacıları desteklemektedir.

Abstract

Aim: The aim of this study is to evaluate the diagnostic criteria and effect of ureteroceles on the nephrourinary systems in our patients. Method: The patients treated for ureterocele between the years 1996-2000 were evaluated retrospectively. The clinical presentation, sex, symptoms, age at diagnosis, radiological diagnostic tools [Vesicocystourethrography (VCUG), intravenous urography (IVU), ultrasound examination (USG)], the results of the Dimercaptosuccinic acid (DMSA) scintigraphies and treatment modalities were investigated. Results: Eight patients, aged between 1 day and 9 years, were evaluated. Six patients were symptomatic due to urinary tract infections (UTI). Vesicoureteric reflux (VUR) was detected in the VCUGs of five patients and the diagnosis of ureterocele was confirmed in 3 series. An IVU revealed ureterocele in six out of seven patients. In the DMSA scintigraphies, one patient had an atrophic kidney whereas the other four patients had scarred kidneys. By ultrasonography, ureterocele was diagnosed in six out of eight patients. On the ureterocele side, six patients showed differentiating degrees of hydroureter, hydronephrosis, and pelvicalyceal dilatation whereas five patients had pelvicalyceal and ureteral dilatations in contralateral renal units. As the surgical treatment three patients underwent upper pole heminephrectomy while bladder level reconstruction was added to this procedure in 2. Two patients underwent only ureterocele excision. In three patients we performed ureterocele excision and ureteral reimplantations. The histopathological examination of three kidney specimens displayed chronic pyelonephritis in all. Five patients showed no problems during the follow up period. One died, and two patients are still under control for the treatment of hypertension and occasional UTI. Conclusion: As a result, we can suggest the importance of UTI as a clue for the diagnosis in older children with ureteroceles. Although the diagnosis is rather easy for ureteroceles, it is a kind of complex disease affecting the whole nephrourinary system even at the diagnostic stage. Our results are in accordance with others that the variety of presentations and anomalies dictate individualized treatment in ureteroceles.
