Özet

Brusellozis Türkiye'de insan ve hayvan sağlığı için önemli bir risk oluşturmaktadır. Bu çalışmada, brusellozisin sadece hayvan sağlığı ve üretimi üzerine olan ekonomik etkileri hesaba katılmış, insan sağlığı ve uluslar arası ticaretle ilgili etkileri hesaplamaya dahil edilmemiştir. Brusellozisin kontrol ve eradikasyonu ile ilgili 4 kontrol stratejisi, 20 yıllık bir periyot halinde, hastalığın gelecek prevalansını tahmin etmek ve fayda/masraf (F/M) analizini hesaplamak için deterministik bir model olarak uygulandı. "Hiçbir şey yapılmaması" stratejisi ile, brusellozis prevalansı ve infeksiyon sebebiyle oluşan kayıplar her yıl hızla yükseleceğinden bu stratejinin tavsiye edilmesi mümkün değildir. Brusellozisin kontrolü için aşılama en yüksek F/M oranına (6.77) sahiptir ve test ve kesim stratejilerinden daha ucuzdur. Aşılama, Türkiye'de ülke geneli için değerli bir uygulama olarak kabul edilebilir. Fakat eradikasyon sadece aşılama programı ile mümkün değildir. Aşılama programı ile birlikte veya tek başına test ve kesim stratejisinin uygulaması, birden küçük (0.77 ve 0.62) F/M oranına ve yüksek toplam masrafa sahiptir ve negatif olan net bugünkü değer ile iç verim oranlan da sıfıra çok yakındır (0.05 ve 0.07). Test ve kesim stratejileri çok masraflıdır. Brusellozisin eradikasyonunun, Trakya gibi küçük bölgelerde yapılması ve özellikle sürü bazında, süt sığırcılığında öncelikli olarak uygulanması ile daha başarılı olacağı düşünülmektedir.

Abstract

Brucellosis is a considerable risk for the human and animal health in Turkey. In this study, the economic effects of brucellosis related to human health and international trade were not investigated. Only the economic effects of brucellosis on animal health and production were considered. Four control strategies of control or eradication of brucellosis were modelled over a twenty year period to predict future prevalence and to calculate benefit / cost analysis using a deterministic spreadsheet model. With the 'do nothing' strategy, prevalence of brucellosis and loss due to infection will increase sharply every year. Therefore it is not possible to recommend this strategy. Brucellosis control by vaccination has the highest benefit / cost ratio (BCR) (6.77) and is cheaper than test and slaughter strategies. Vaccination could be a worthwhile procedure in Turkey for the whole country. Eradication is not possible with only vaccination programme. Using test and slaughter strategy without and with vaccination programme have the BCR of less than one (0.62 and 0.77), high total cost, negative net present value (NPV) and internal rate of return (IRR) which is very close to zero (0.05 and 0.07). Test and slaughter strategies are extremely costly. It could be possible to eradicate brucellosis from small parts or regions of Turkey such as Thrace region. Eradication of brucellosis from dairy herds could also be considered as a priority.
