Özet

Amaç: Bu çalışmada hastanemizde vena kava superior sendromu (SVCS) nedeniyle takip edilen hastaların patolojik tanılarını, tanı yöntemlerini, semptomlarını, ortalama yaşam sürelerini değerlendirmeyi amaçladık. Gereç ve Yöntem: Dört yıllık sürede izlenen tanısı biopsi ile doğrulanmış olan akciğer kanseri, tüberküloz, lenfoma, Behçet Hastalığı etyolojili 25 olguluk seride, eşlik eden SVCS bulunan bu hastaların verileri gözden geçirildi. Bulgular: Yirmibeş olgunun büyük çoğunluğunu akciğer kanserli olgular oluşturmaktaydı (%80). Olgularda en sık gözlenen semptomlar yüzde şişme (%44) ve dispne (%64) olarak bulunmuştur. Akciğer grafisinde olguların %44'ünde üst mediastende genişleme görülmüştür. Hastaların patolojik tanısı trans-torakal ince iğne biopsisi (TTİAB) (%12), supraklavikuler lenf bezi biopsisi (%8), fiber-optik bronkoskopi (FOB) (%48), mediastinoskopi (%20), manyetik rezonans (%4), torasentez (%4), balgam sitolojisi (%4) ile konulmuş olup bu girişimler sırasında komplikasyon oluşmamıştır. Çalışmamızda small cell akciğer kanserli olguların ortalama yaşam sürelerinin (7.3 ay) non-small cell akciğer kanserli olgulardan (4.4 ay) uzun olduğu görüldü. Sonuç: Sonuç olarak SVCS'lu olgularda etyolojiyi belirlemede mediastinoskopi, torasentez, FOB, TTİAB, lenf bezi biopsisinin güvenli bir şekilde uygulanabileceğine ve small cell akciğer kanserli olguların non-small cell akciğer kanserli olgulara göre daha uzun yaşadığı kanısına varıldı.

Abstract

Objective: The aim of this study was to analyze the features of the superior vena cava syndrome (SVCS): incidence, symptoms, diagnostic procedures, median survival in patients with SVCS. Material and Method: In a series of 25 patients with biopsy proven lung cancer, tuberculosis, lymphoma, Behçet disease seen during a 4-year period, we reviewed data from patients who also had SVCS. Result: Of the 25 cases lung cancer constituted the majorty of the cases (80%). Our results showed that dyspnea and facial swelling cases were the most common symptoms (64% and 44%). The chest X ray finding also showed a large ratio of superior mediastinal widening (44%). Fine needle aspiration biopsy (12%), lymph node biopsy (8%), broncoscopy (48%), mediastinoscopy (20%), sputum cytology (4%), toracentesis (4%), magnetic resonance (4%) made diagnosis of cases. With these methods no complication was recorded. There were a longer survival time in small cell lung cancer (7.3 months) as compared to that of non-small cell cancer (4.4 months). Conclusion: We concluded that lung cancer with SVCS could be safely diagnosed by needle aspiration biopsy, lymph node biopsy, mediastinoscopy, toracentesis, broncoscopy in SVCS patients and small cell lung cancer patients survived longer than non-small cell lung cancer patients.
