Özet

Kolonun Crohn hastalığında total proktokolektomi düşük nüks oranlarıyla beraberdir, ancak hastalara kalıcı bir ileostomi sunar. Segmenter kolonik rezeksiyon ise, nüks ve tekrarlayıcı girişimlerin yüksek oranıyla birliktedir fakat seçilmiş olgularda, hastayı bir stomadan koruyabilir veya stoma formasyonunu geciktirebilir. Bu çalışma sınırlı kolonik Crohn olgularında kolon koruyucu prosedürlerin, gastrointestinal devamlılığı sağlamada efektif olup olmadığını tayin etmek için planlandı. Kliniğimizde 1988-1997 yıllan arasında sınırlı Crohn kolitisi nedeniyle segmenter kolonik rezeksiyon uygulanan 8 olgu çalışmaya dahil edildi. Lezyon 2 olguda transvers kolona, 2 olguda sol kolona, 4 olguda ise rektuma lokalizeydi. Ortalama 50 aylık bir izlem sonrası 6 hasta halen hastalıksızdı ve bu dönem içinde, spesifik hastalığına yönelik herhangi bir tedavi gerektirmemişti. Bir hasta rekurrense bağlı . enterokütan flstül ve renal amiloidozis nedeniyle önce medikal olarak sonrada subtotal kolektomi ile tedavi edildi. Bir hastaya ilk operasyonun 2. yılında ortaya çıkan ileoçekal hastalık ve buna bağlı enterokütan fıstül nedeniyle ileoçekal rezeksiyon uygulandı. Sınırlı kolonik Crohn olgularında proktektomi olmaksızın segmenter kolon rezeksiyonun kalıcı stoma gereksiniminden kaçınmak veya geciktirmek amacıyla uygun her seçenek olduğu kanısındayız.

Abstract

Total proctocolectomy and ileostomy for Crohn's colitis offers a low recurrence rate but commits patients to a permanent ileostomy. In contrast segmentai resection may predispose patients to recurrence and further surgery but may delay or avoid a stoma in selected individuals. This study was undertaken to determine whether colon-sparing procedures were effective at preserving gastrointestinal continuity in cases of limited colonie disease. This study includes 8 patients underwent segmentai colonie resection for limited Crohn's colitis at the our clinic between 1988 and 1997. All of them were alive and available for follow-up. Crohn's of the colon involved the tranvers colon, left colon and rectum in 2 , 2 and 4 patients, respectively. During a mean follow-up of 50 months,six patients required no further therapy. In 2 patients, further treatment was required, including 1 patient who was managed medically then surgically and 1 patient who was managed surgically. Colon resection without proctectomy in selected patients with limited colonie Crohn's disease can delay or avoid the necessity of a permanent stoma.
