Özet
Tiroid nodüllerinin ameliyat öncesi benign ve malign ayırıcı tanısında ince iğne aspirasyon biyopsisinin (İlAB) etkinliğini araştırmak amacıyla yapılmış bir çalışmadır. Tiroid nodulu sebebiyle ameliyat edilen 57 hastanın İİAB ile elde edilen sitolojik inceleme sonuçlarıyla, ameliyattan sonraki histopatolojik değerlendirme sonuçlan karşılaştırıldı. Yetersiz değerlendirme oram %5.3 idi. Beş İİAB sonucu malign iken, bunlardan 2 hastada malignité histopatolojik olarak doğrulandı. Histopatolojik olarak toplam 4 hastada malignité belirlenirken, bunlardan 2 tanesinin sitolojik değerlendirmesi benign idi. Duyarlılık, seçicilik, pozitif prediktif değer, negatif prediktif değer ve tanısal doğruluk oranları sırasıyla %50, %94, %40, %96, %90.7 olarak belirlendi. Sonuç olarak, İİAB tiroid nodüllerinin tanısında kullanılabilecek bir yöntemdir. Duyarlılığı ve pozitif prediktif değeri düşük olduğundan, sadece benign İİAB sonuçlarına bakarak ameliyattan vazgeçmek uygun değildir.
Abstract
The aim of this study was to investigate the confidence of fine needle aspiration biopsy (FNAB) in the preoperative diagnosis of benign and malignant thyroid nodules. FNAB cytology results were compared with histopathologic results of 57 operated patients with thyroid nodules. The rate of inadequate specimens was 5.3%. Five aspiration biopsies were positive for malignancy, and the diagnosis was confirmed histopathologically in 2 of them. Two of 4 patients with proven carcinoma had benign cytological diagnosis. The sensitivity, the specificity, the positive predictive value, the negative predictive value and the diagnostic accuracy of this method were determined as 50%, 94%, 40%, 96%, 90.7% respectively. As a conclusion FNAB can be used as a diagnostic method in the evaluation of thyroid nodules. Since the sensitivity and the positive predictive value are low, it is not suitable to decide the cancellation of the operation according to only benign FNAB results.
