Özet
İntrauterin gelişine geriliği (İUGG) perinatal morbidité ve mortalité açısından erken tanısı oldukça önemli olan klinik bir durumdur. İUGG tanısında pek çok fetal ölçümün kullanıldığı çalışmalar vardır. Bu çalışmada yeni bir test olan fetal pondéral indeksin İUGG ' nin tanısında ve asimetrik İUGG ' nin ayrımında etkinliğini araştırarak, tahmini doğum ağırlığı ile karşılaştırdık Fetal pondéral indeks Vintzileous ' un formülü ile hesaplandı. Çalışma yüksek riskli 49 hastada gerçekleştirildi. 32 hastada doğum ağırlığı % 10 persentilin altında olduğu için İUGG tanısı konuldu. Fetal pondéral indeksin sensivitesi % 56,3, spesifltesi %94, l,pozitif prediktiv değeri %94,7, negatif prediktiv değeri %53,3 olarak bulundu. Hastalar neonatal pondéral indeksleri hesaplanarak asimetrik ve simetrik İUGG olarak ayrıldıklarında fetal pondéral indeksin asimetrik gelişme geriliğinin tanısında spesifıtesinin ve pozitif prediktivitesinin tahmini fetal ağırlığa göre daha yüksek olduğunu bulduk. Fetal pondéral indeksin İUGG ' nin antenatal teşhis ve takibinde kullanılabilecek testlerden biri olduğu kanısındayız.
Abstract
Accurate and early diagnosis of Intrauterine Growth Retardation (IUGR) decreases perinatal morbidity and mortality. There are many tests being used for the diagnosis of IUGR. In this study a recently proposed test, Fetal Pondéral Index (FPI) was investigated for the diagnosis of IUGR and also for the discrimination of the symmetric and the asymmetric types of IUGR and a comparison was made between the FPI and estimated fetal weight (EFW) for the diagnosis of IUGR. FPI was calculated by using Vintzileous formula. Fourty nine high risk patients were enrolled into this prospective study. In 32 patient IUGR was diagnosed on the basis of fetal weight being lower than the tenth percentile for gestational age. In diagnosis of IUGR the sensitivity, specificity, positive predictive value and the negative predictive value for FPI was calculated as 56,3 %, 94,1% ,94,7% and 53,3% respectively. After Neonatal Pondéral Index measurements, patients were grouped as either having symmetric or asymmetric IUGR. In the asymmetric IUGR group the specificity and positive predictive value of FPI was found to be higher in comparision to EFW measurements. As a result, we think that FPI is a valuable test in the diagnosis of IUGR.
