Özet

AMAÇ: Gastrointestinal kanserlerde malign hücrelerin abdominal sıvı sitolojisi incelemesindeki insidansı ve bunun klinik önemi halen tartışmalıdır. Bu çalışmada mide ve kolon kanserli hastaların periton sıvısında serbest kanser hücrelerinin tespit edilme sıklığı ve klinik önemi araştırılmıştır. GEREÇ VE YÖNTEM: 1994-1998 yılları arasında mide kanseri olan 53 hasta ve kolon kanseri olan 36 hasta çalışmaya alınmıştır. Laparotomiden sonra eğer asit mevcutsa örnek alınmıştır. Asit olmayan hastalarda ise 50 ml serum fizyolojik ile subhepatik, subdiyafragmatik ve pelvik bölge irrige edilmiş, irrigasyon sıvısı geri alınmıştır ve sitolojik incelemeye gönderilmiştir. Farklı tümör evrelerindeki sitoloji pozitifliği irdelenmiştir. BULGULAR VE SONUÇ: Mide kanseri olan 53 hastanın 14'ünde (%26.4) ve kolon kanseri olan 36 hastanın 6'sında (%16.6) sitolojik inceleme pozitif olarak bulunmuştur. Her iki grupta da sitolojik inceleme sonucu ile tümör evresi arasında istatistiksel olarak anlamlı bir korelasyon tespit edilmemiştir. Sitolojik incelemenin pozitif bulunması ile prognoz arasında bir ilişki olduğuna inanmaktayız. Peritoneal sitolojik incelemenin standardizasyonu ve yaygın kullanımı ile artan veri birikimi, tümör davranışının anlaşılmasında ve bunun prognoz üzerindeki etkilerini ortaya koymada son derece önemli bilgiler verecektir.

Abstract

PURPOSE: The incidence of malignant cells on abdominal fluid cytology in gastrointestinal cancers and its clinical validity has not been well-defined. This study was designed to delineate the frequency and significance of free cancer-cells in the peritoneal cavity of patients -with stomach and colonic cancer. MATERIAL AND METHODS: Between 1994-1998; 53 patients with gastric cancer and 36 patients with colonic cancer underwent laparotomy. When the abdominal cavity was opened, any ascitic fluid present was collected. A plastic syringe containing 50 ml sterile saline was used to irrigate subhepatic, subdiaphragmatic and pelvic areas, and the fluid was recollected. The cytologic examination was performed by the cytologist on duty. The positivity of the cytologic examination at different tumour stages was analysed. RESULTS AND CONCLUSION: The cytological examinations revealed positive results in 14 of 53 gastric cancer , patients (26.4%) and 6 of 36 colonic cancer patients (16.6%). Statistical analyses showed no correlation between cytological results and tumour stage in both gastric and colonic cancer group. We believe that in intraabdominal malignancies, the positivity of cytologic examination might give a valuable information about the prognosis of the disease. In the future, in intraabdominal malignancies, the standardization and widely usage of this technique would improve our data for understanding the tumour behaviour and its effect on prognosis.
