Özet
AMAÇ: Outpatient anestezi ve cerrahi son yirmi yılda birçok avantajından dolayı ilgi odağı haline gelmiştir. Outpatient anestezi ve cerrahi programına alınan voltaların sayısı ve çeşidi giderek artmakladır. Günümüzde meme biyopsisi uygulanacak vakalar outpatient programdadır. GEREÇ VE YÖNTEM: Biz çalışmamızda meme biyopsisi uygulanacak hastalara etkin sedasyon, analjezi ve amnezi sağlamayı amaçladık. ASA I-II grubunda 30 hasta rastgele 2 gruba ayrıldı. Grup-I'deki hastalara (n=15) indüksiyonda l mg/kg ketamin ile 0.01 mg/kg midazolam I.V. uygulandı. İdamede 0.5 mg/kg/saat ketamin ile (0.05 mg/kg/saat midazolam infüzyonu yapıldı. Grup II'deki hastalara (n=15) ise, indüksiyonda 1-2 mg/kg propofol, ardından 50-200 mcg/kg/saat propofol infüzyonu uygulandı. Respiratuar depresyonun derecesi ve Ramsey Sedasyon Skoru ile sedasyon değerlendirildi. Ayrıca hasta, anesteziyolog ve cerrah tarafından yöntemin etkinliği global olarak değerlendirildi. BULGULAR VE SONUÇ: Sonuçla, ketamin+midazolam uygulanan grupta propofol uygulanan gruba göre daha hızlı sedasyon sağlandı. Hasta, anesteziyolog ve cerrah tarafından değerlendirilen global değerlendirme skorlarında ketamin +midazolam grubu propofol grubundan yüksek puan aldı.
Abstract
OBJECTIVE: Outpatient anesthesia and surgery have become more interesting in recent years. The number and type of patients including the programme of outpatient anesthesia and surgery are increasing. Mammary biopsy patients are included in the outpatient programme. MATERIAL AND RESULTS: The purpose of this study is to maintain sedation, analgesia and amnesia during mammary biopsy. Thirty patients (ASA I-II) were allocated into two groups randomly. In Group I, patients (n:=15) received 1 mg/kg ketamine and 0.01 mg/kg midazolam (I V} for induction of anesthesia, while the patients in Group II (n=15) were given 1-2 mg/kg propofol for the induction and 50-200 mcg/kg/hour propofol infusion for the maintanance of anesthesia. Degree of respiratory depression and sedation using Ramsey Sedation Score were evaluated. Besides, the evaluation of efficacy of technique by the patient, anesthesiologist and surgeon. CONCLUSION: In conclusion, sedation was faster in the ketamine-midazolam combination group when compared with propofol. In the ketamine and midazolam group, the overall evaluation scores assessed by the patient, anesthesiologist and surgeon were higher than the propofol group.
