Clark level IV cilt malign melanomunda profilaktik lenf nodu diseksiyonunun sağkalım üzerine etkisi

AMAÇ: Çalışmada Clark Level IV cilt malign melanomunda profilaktik lenf nodu diseksiyonunun sağkalım üzerine olan etkisini araştırmayı amaçladık. GEREÇ VE YÖNTEM: 1990-1996 yılları arasında histopatolojik olarak Clark Level IV cilt malign melanomu tanısı konulan 56 hastaya rejyonal lenf nodu diseksiyonu uygulandı. Klinik olarak lenf nodu yokluğunda diseksiyon profilaktik, varlığında ise terapötik diseksiyon (TLND) olarak kabul edildi. PLND grubunda 29 hasta (%51.8), TLND grubunda ise 27 hasta (%48.2) mevcuttu. Her iki grup, cinsiyet, yaş, lokalizasyon, metastatik lenf nodu sayısı, rejyonal nodal nüks, lokal nüks, uzak metastaz, 3 yıllık ve 5 yıllık sağkalım parametrelerine göre değerlendirildiler. BULGULAR: Her iki grupta incelenen parametreler açısından saptanan metastatik lenf nodu sayısı dışında anlamlı bir fark yoktu. PLND grubunda ortalama sağkalım 56.7 ay, 3 yıllık ve 5 yıllık sağkalım sırası ile %88.5 ve %54.0 iken TLND grubunda ortalama sağkalım 67.1 ay ve 3 yıllık ve 5 yıllık sağkalım sırası ile % 73.0 ve %58.8 idi. Aradaki fark istatistiksel olarak anlamsızdı (p=0.856>0.05). SONUÇ: Clark Level IV malign melanomlu hasta grubumuzda profilaktik lenf nodu diseksiyonu ile sağkalım avantajı elde edilmemiştir.
Abstract
OBJECTIVE: The aim of the study was to determine the survival effect of PLND in Clark Level IV malignant melanomas of the skin. MATERIAL AND METHODS: 56 histopathologically proven Clark Level IV melanoma patients underwent regional lymph node dissection between the years of 1990-1996. When the regional lymph node(s) were clinically negative, dissection was accepted as prophylactic, when positive accepted as therapeutic. There were 29 patients (51.8%) in PLND group and 27 (48.2%) in TLND group. Two groups of cases were compared for age, localisation, number of metastatic lymph nodes, regional nodal recurrence, local recurrence, distant metastasis, 3- and 5- year overall survival. RESULTS: There was no statistically significant difference between the two groups for the evaluated parameters except for the number of metastatic lymph nodes. In PLND group, the mean survival was 56.7 months, 3- and 5- year overall survival was 88.5% and 54.0% respectively. In TLND group the mean survival was 67.1 months, 3- and 5- year overall survival was 73.0% and 58.8% respectively. The survival difference was not statistically significant (p=0.0856>0.05). CONCLUSION: In our Clark Level IV malignant melanoma patient group PLND has shown no survival benefit.
