Refrakter hiperemezis gravidarum'un tedavisinde oral metilprednisolon'un yeri

Özet
AMAÇ: Konvansiyonel tedaviye dirençli gebelik kusmalarının sağaltımında oral Metilprednisolon'un etkisini inceledik. GEREÇ VE YÖNTEM: SSK Ege Doğumevi'nde Eylül 1998- Haziran 2000 tarihleri arasında hiperemezis gravidarum tanısı konup alışılagelmiş tedavilere yanıt vermeyen gebeler çalışmaya alındı. Çoğul gebelik, fetal veya plasental anomalili, şüpheli veya kanıtlanmış hastalığı olan gebeler çalışma dışı bırakıldı. 8 ile 20 hafta arasında refrakter hiperemezis gravidarum tanısı alan 30 hastaya 3 gün boyunca günde 48 mg oral metilprednisolon verildi. Doz iki hafta boyunca azaltılarak kesildi. İki hafta sonunda tedaviye yanıt vermeyen veya yakınmaları yeniden başlayan gebelere aynı dozda tedavi tekrar başlandı ve 1 aylık süreden uzun olmamak üzere tedaviye devam edildi. BULGULAR: Otuz hastanın yirmibeşinde (%83.3) bulantı ve kusma şikayetleri kesildi, ve 3 gün içinde düzenli olarak dietlerini almaya başladılar. Yirmibeş hastanın ondördünde (%56) gebelikleri boyunca herhangi bir şikayet tespit edilmedi. Dokuz hastada (%36) tedavi sonrası veya doz azaltımı sırasında şikayetler tekrarladı. Bu hastaların tamamı tedavinin uzatılmasına cevap verdi. İki hasta (%8) ise ancak uzatılmış tedavinin 28-29.'uncu günü tedaviye yanıt vermeye başladı ve bunlarında gebelikleri süresince şikayetleri tekrarlamadı. Geri kalan beş hastanın birinde metilprednisolon'a intolerans gelişirken, üçünde uzatılmış tedavinin doz azaltımı sırasında rekürrens tespit edildi, kalan diğer hastada ise herhangi bir şekilde tedaviye yanıt alınamayarak, genel durum bozukluğu sebebiyle total parenteral nütrisyon uygulandı. SONUÇ: Kısa süreli oral Metilprednisolon tedavisinin refrakter hiperemezis gravidarumun sağaltımında anlamlı bir alternatif olduğu düşünülmektedir.
Abstract
OBJECTIVE: Our purpose was to evaluate the effect of oral methylprednisolone on the course of refractory hyperemesis gravidarum. MATERIAL AND METHODS: Patients with the diagnosis' of hyperemesis gravidarum who were admitted to SSK Aegean Maternity Hospital in between September 1998 and June 2000 and refractory to convansional treatment modalities were included into the study. Patients with multiple gestations, fetal or placental anomalies and suspected or proven medical or surgical diseases that could have explained the vomiting were not offered this treatment. 30 patients with intractable hyperemesis gravidarum were admitted to the hospital. 48 mg oral methylprednisolone per day for three days were given to these thirty patients with gestations in between 8 and 20 weeks. The dosage was tapered over 2 weeks. The treatment protocol was again given to patients who were refractory to this treatment or to patients with recurrence of vomiting symptoms at the end of these two weeks. And the treatment was continued for not over one month. RESULTS: Twenty-five of thirty patients (83.3%) were free of vomiting and were able to tolerate a regular diet within 3 days of starting methylprednisolone therapy. Fourteen of twenty-five patients (56%) didnot have recurrence of vomiting for the remainder of the pregnancy. Recurrence of symptoms was observed in nine patients (36%) after the therapy or during the tapering of the dosage period. All of these nine patients responded to prolongation of the therapy. Two patients (8%) responded to the therapy at 28th-29th days of the prolonged therapy and these two patients were also free of symptoms for the rest of the pregnancy. From the remaining five of thirty patients; one developed intolerance to methylprednisolone, three patients developed recurrence of the symptoms during the tapering phase of prolonged therapy and the last patient didnot respond to any of the therapies and total parenteral nutrition was begun because of generalized depressed condition. CONCLUSION: A short course of oral methylprednisolone appears to be a reasonable therapeutic alterantive for intractable hyperemesis gravidarum.
