Özet
AMAÇ: Bu çalışmada kliniğimizin Milligan-Morgan hemoroidektomi sonuçları değerlendirilmiştir. Hemoroid genel cerrahinin yaygın hastalıklarından biridir. 1. ve 2. derece hemoroidlerde konservatif tedavi yöntemleri genellikle yeterli olabilirken 3. ve 4. derece hemoroidler ve kanamalı hemoroidlerde çoğunlukla hemoroidektomi endikasyonu doğmaktadır. GEREÇ VE YÖNTEM: Bu çalışmada, 1994-2000 yılları arasında Ankara Numune Eğitim ve Araştırma Hastanesi 3. Cerrahi Kliniği'nde opere edilen 293 vaka retrospektif olarak değerlendirildi. BULGULAR: 293 hastanın 196' sı erkek (%66.9), 97' si (%33.1) kadındı, ortalama yaş 43.9 idi. Bu hastaların 270'ine (%92.1) Milligan-Morgan hemoroidektomi, 15 olguya (%5.1) Milligan-Morgan+Reclus, 6 olguya (%2.1) Reclus, 1 olguya da (%0.6) Whitehead tekniği uygulandı. Milligan-Morgan yöntemi için ortalama ameliyat süresi 25 dakika, hastanede kalış süresi 3 gündü. 15 hastada erken komplikasyon (hemoraji, üriner retansiyon), 4 hastada geç komplikasyon (mukozal ektropion, anal stenoz) oluştu, nüks görülmedi. SONUÇ: Milligan-Morgan hemoroidektomi tekniği; kolay uygulanan, kısa süreli, başarı oranı yüksek, ciddi komplikasyonları az olan bir operasyondur.
Abstract
OBJECTIVE: This study was carried out to evaluate our clinics' experience to hemorrhoids comparing with the literature. Hemorrhoids are widespread pathology that are seen most in general surgical practice. Conservative surgical methods are used commonly in first and second degree hemorrhoids. Hemorrhoidectomy is indicated in third, fourth degree hemorrhoids and bleeding hemorrhoids. MATERIALS AND METHODS: In this study, 293 patients who underwent operation between 1994 and 2000 years in the third surgical clinic of Numune Teaching and Training Hospital, are evaluated retrospectively. RESULTS: Out of 293, 196 were men (%66.9), 97 woman (%33.1) and mean age was 43.9. Milligan-Morgan hemorrhoidectomy method was employed in 270 patients (%92.1), Milligan-Morgan+Reclus in 15 (%5.1), Reclus in 6 (%2.1) and Whitehead in 1 (%0.6). Mean operative time was 25 minutes and hospital stay 3 days for Milligan-Morgan method. While early complications (hemorrhage, urinary retention) were seen in 15 patients, late complications (Anal mucosal ectropion, anal stenosis) in four. We had no recurrent disease . CONCLUSION: Consequently, Milligan -Morgan hemorrhoidectomy is a technique that can be employed easily with a short operative time and that has high successful rate, less serious complication.
