Hipotermik yenidoğanlarda kanamaya eğilim

Özet
Amaç: Bu çalışmada hipotermi ile başvuran yenidoğanların hematolojik parametreleri kontrol grubuyla karşılaştırılarak hipoterminin kanamaya eğilim meydana getirip getirmediği araştırıldı. Gereç ve Yöntem: Çalışma Yenidoğan Yoğun Bakım Ünitesinde Haziran 2000 ile Şubat 2002 tarihleri arasında yapıldı. Hipotermi saptanan ve yaşamın ilk saatlerindeki 49 yenidoğan çalışma grubu olarak alındı. Hastaların üniteye geliş ağırlığı, gestasyon yaşı, cinsiyeti, hastanede kalış süresi ve sonucu kaydedildi. Gelişte ve 0,1,2,3,4,6,8,10,12. saatlerdeki cilt ısısı ölçüldü. Yatıştan ortalama 3 saat sonra tam kan sayımı, pıhtılaşma testleri (protrombin zamanı ve aktive parsiyel tromboplastin zamanı) ve doğal koagülasyon inhibitörleri (protein C, protein S ve antitrombin III) ölçüldü. Bulgular: Her iki grubun da gestasyonel yaş (35.3±3.9 hafta ve 34.2±3.7 hafta), ağırlığı (2170±809 gr ve 1896±801 gr) ve cinsiyet dağılımları (erkek %63.3 ve %75) birbirine benzerdi (p>0.05). Protrombin zamanı ve aktive parsiyel tromboplastin zamanı hipotermik grupta anlamlı olarak daha uzundu (sırasıyla, 26.9±10.4 sn ve 15.9±1.1 sn; 62.7±23.3 sn ve 39.2±6.8 sn) (p<0.05). Protein C, protein S ve antitrombin III değerleri hipotermi grubunda anlamlı olarak düşüktü. Hipotermik bebeklerin lökosit, D-dimer, alanin aminotransferaz ve aspartat aminotransferaz değerleri daha yüksek bulundu (p<0.05). Hipotermik yenidoğanlarda mortalite daha fazlaydı (p<0.05). Sonuç: Yenidoğan döneminde hipoterminin hematolojik parametrelerde dengesizliğe yolaçarak morbidite ve mortaliteyi arttırdığı sonucuna varıldı. Hipotermik bebeklerin hematolojik parametrelerinin yakından izlemi ve düzeltilmesi ile morbidite ve mortalitenin azalabileceğini düşünmekteyiz.
Abstract
Objective: In this study we studied the hematologic parameters of hypotermic neonates and compared then with controls in order to investigate whether hypothermia itself makes hemorrhagic diathesis. Material and Methods: This study has been conducted in Neonatal Intensive Care Unit between June 2000 to February 2002. Forty-nine newborns who had hypothermia and were admitted in first hours of their life were assessed within the context of the study group. Admission weight, gestational age, gender, hospitalization time and outcome of patients were recorded. Skin temperature was measured on admission, 1,2,3,4,6,8,10, and 12th hours. Complete blood analysis, clotting tests (prothrombin time and activated partial thromboplastin time) and natural cogulation inhibitors (protein C, protein S and antithrombin III) were studied 3 hours after admission. Results: Gestational age (35.3±3.9 wk vs 34.2±3.7 wk), weight (2170±809 g vs 1896±801 g) and gender distribution (male 63.3% vs male 75%) were similar in both groups (p>0.05). We found that protrombin time and activated partial thromboplastin time were much longer in hypothermic group when we compared to control group (26.9±10.4 s vs 15.9±1.1 s; 62.7±23.3 s vs 39.2±6.8 s, respectively) (p<0.05). The protein C, protein S, and antithrombin III counts were significantly lower in hypothermic group. However, in hypotermic newborns leucocyte, D-dimer, aspartat aminotransferase and alanine aminotransferase counts were significantly higher than controls (p<0.05). Mortality rate was higher in hypothermic newborns (p<0.05). Conclusion: Finally, we found that the hypothermia itself increases morbidity and mortality in the neonatal period by disturbing hematologic parameters. We have been excogitating that close following and improving of hemorrhagic parameters of hypothermic newborns may decrease morbidity and mortality rate.
