Özet

Amaç: Gestasyonel Diabetes Mellitusun tedavisinde glibenclamide ile insulinin glisemik kontrol, maternal ve fetal sonuçlar açısından karşılaştırmak. Gereç ve Yöntem: Ocak 2000-Şubat 2002 tarihleri arasında gestasyonel diabetes mellitus tanısı alan gebelikleri 11-33 hafta arası değişen ve ilaç tedavisi gereken toplam 24 tekiz gebelik çalışmaya dahil edildi. Birinci dönem ilk gruba insulin (n=15), ikinci dönem ikinci gruba glibenclamide (n=9) verildi. Her iki grup glisemik kontrol, SGA, LGA, makrozomi, sezeryan oranları, neonatal metabolik sonuçlar, perinatal morbidite ve neonatal sonuçlar açısından karşılaştırıldı. Devamlılık içeren değişkenler için Mann Whitney U-test, kategorik değişkenler için de yerine göre ki kare veya Fisher's exact test kullanıldı. Alfa için eşik değer 0.05 kabul edildi. Bulgular: Her iki grup demografik açıdan benzerdi. AKŞ'i, postprandial 2. saat sabah ve öğle değerleri glibenclamide lehine belirgin düşüktü (p=0.018, p=0.012, p=0.001). Her iki grup arasında LGA, SGA, sezeryan oranları, doğum ağırlıkları, doğumda gebelik yaşları, Apgar skorları, perinatal sonuçlar ve neonatal metabolik sonuçlar açısından istatistiksel anlamlı fark izlenmedi. Sonuç: Glibenclamide iyi bir glisemik kontrol sağlamasının yanında insulin ile karşılaştırılabilir perinatal ve neonatal sonuçlara sahiptir.Gelecekte geniş serili prospektif randomize çalışmalar ile desteklendiği taktirde gestasyonel diabetes mellitusun tedavisinde ümit vaad etmektedir.

Abstract

Objective: To compare te efficiency of glibenclamide versus insulin in achieving euglycemia in cases of gestational diabetes mellitus. Materials and Methods: 24 singleton pregnancies between 11-33 weeks of gestation with the diagnosis of gestational diabetes unresponsive to dietary measures were included in this study. During the time period 1.10.2000-1.12.2001 15 patients received insulin and during the time period 2.12.2001-31.12.2002 9 patients received glibenclamide for gylcemic control. Both groups then were compared in terms of glycemic control, rates of SGA, LGA, macrosomia, caesarean section, perinatal morbidity and unfavorable neonatal outcomes. Mann Whitney U test for continuous and chi square or Fisher's exact test for cathegorical variables were used where appropriate. Cut off for alpha was set at 0.05. Result: Both groups were similar in demographical characteristics. Fasting glucose levels and postprandial 2 hour morning and evening levels were significantly lower in glibenclamide group (p=0.018, p=0.012, p=0.001). There was no statistical significant difference between both groups in terms of LGA, SGA, cesarean rates, Apgar scores, birth weights, perinatal outcome and neonatal metabolic results. Conclusion: In this preliminary study glibenclamide was proven to be at least as efficient as insulin in achieving euglycemic control in cases of gestational diabetes unresponsive to dietary measures. If proven by further large scale multicentric prospective studies, then glibenclamide might establish itself as a first choice agent in the medical management of gestational diabetes.
