Kronik C hepatitli 'Naive' hastalarda interferon-ribavirin standart tedavisinin indüksiyon tedavisi ile karşılaştırılması

Özet
Amaç: Bu çalışmada standart (haftada 3 kez 3 MÜ interferon alfa (IFN-alfa) ve 1000-1200 mg/gün ribavirin (riba) kombinasyon tedavisi ile indüksiyonlu (1 ay süre ile hergün günde 10 MÜ IFN-alfa ve riba 1000-1200 mg/gün) kombinasyon tedavisinin tedavi sonu ve kalıcı cevap yönlerinden karşılaştırmayı ve indüksiyonlu tedavinin bir üstünlüğü olup olmadığını araştırmayı amaçladık. Gereç ve Yöntem: Çalışmaya kronik hepatit C tanısı almış ve daha önce hiç tedavi görmemiş 26 hasta dahil edildi. Bir yıl süreyle 20 hastaya (grup 1) standart kombine tedavi ve 6 hastayada (grup 2) indüksiyonlu kombine tedavi uygulandı. Tüm hastaların tedavi öncesi hematolojik, bioşimik ve virolojik durumları (PCR yöntemi ile) ve histolojik incelemeleri (Knodell skorlamasına göre) belirlendi. Altı aylık aralıklarla bioşimik ve virolojik cevaplar tedaviye yanıt yönünden değerlendirildi. Gruplar tedavi bitimi (12. ay) ve tedavi kesildikten sonraki 6. ayda (18. ay) değerleri açılarından karşılaştırıldılar. Bulgular: Gurup 1de tedavi bitimi hastaların 13/20 (%68.4) sinde ALT değerleri normale gelmiş olup, ortalama ALT değerleri 35.5±5.6 U/L, HCV-RNA 12/20 hastada negatifti (%58.3). Tedavi kesildikten 6 ay sonra ise ALT normalleşmesi 7/20 (%58.3), HCV-RNA negatifliği (PCR ile HCV-RNA negatifliği (PCR ile) %57.1 idi.Gurup 2 de ise, tedavi bitimi hastaların 5/6'sında (%83.3) ALT normalleşmiş olup ortalama ALT değerleri 34.5±11.5 U/L ve HCV-RNA negatifliği 5/6 (%83.3), tedavi sonrası 6. ayda hastaların 4/6'sında (%80) ALT değerleri normal olup ortalama ALT değerleri 31.0±12.3 U/L ve HCV-RNA negatifliği (PCR ile) 4/6 (%66.7) idi (p>0.05). Sonuç: Çalışmamızın sonunda kronik hepatit C'li genotip 1 sahip naive hastalarda standart interferon-ribavirin kombinasyon tedavisi ile karşılaştırıldığında yüksek doz günlük indüksiyon interferon ve ribavirin kombinasyonu arasında tedavi sonu yanıtı ve kalıcı yanıt açısından istatistiksel olarak anlamlı fark saptanmamıştır.
Abstract
Objective: In this study we aimed to investigate standart combination therapy (3 MU IFN- ttw and 1000-1200 mg/day ribavirin) with induction therapy (10 MU IFN- every day for a month and 1000-1200 mg/day ribavirin) according to end of treatment and sustained responses. Material and Method: Twenty-six naive patients with chronic hepatitis C were included to this study. For a year period standart combination therapy was given to 20 patients (group 1), while induction therapy to other 6 patients (group 2). Pre-treatment haematological, biochemical, virological (by PCR) and histological (according to Knodell scores) findings of all patients were determined. To investigate the responses to treatment biochemical and virological parameters of all patients were evaluated with six month periods. Groups were compared according to end of treatment (12. month) and post-treatment (18. month) findings. Results: In group 1 at the end of the treatment in 13 of 20 patients (68.4%) ALT normalisation was observed (mean ALT values were 35±5.6 U/L) and HCV-RNA was negative in 12 of 20 patients (58.3%).At the sixth month of post-treatment period ALT normalisation was observed in 7 of 20 patients (58.3%) and HCV-RNA negativity (with PCR) was 57.1%.In group 2 ALT normalisation was seen in 5 of 6 patients (mean ALT values were 34.5±11.5 U/L) (83.3%) and negativity of HCV-RNA was observed 5 of 6 patients (83.3%). At the post-treatment sixth mont 4of 6 patients (80%) ALT values were still in normal limits (mean ALT values were 31.0±12.3 U/L) and negativity of HCV-RNA was seen in 4 of 6 patients (66.7%) (p>0.05). Conclusion: In this study, statistically no significant differences was found between the naive chronic hepatitis C patients treated with standart inerferon-ribavirin combination therapy and with high dose inerferon-ribavirin combination therapy.
