Yenidoğan sepsisinin erken tanısında fibronektin, haptoglobin, seruloplazmin ve transferrin'in önemi

Özet
Yaş, cins ve ağırlıkları itibariyle benzer olan, kültür pozitif 24 sepsisli yenidoğan bebek ile sağlıklı 17 bebekte fîbronektin, haptoglobin, seruloplazmin ve transferrin konsantrasyonları ölçüldü. Ayrıca, total lökosit sayısı, total nötrofıl sayısı ve band/nötrofıl oranları tesbit edildi. Fîbronektin ve haptoglobin konsantrasyonları sepsisli ve normal grupta önemli ölçüde farklı idi (p<0.05). Seruloplazmin ve transferrin düzeyleri ise her iki grupta benzer bulundu. Lökosit formülünde sayılan total nötrofıl sayısı ve band/nötrofıl oranları sepsis grubunda önemli ölçüde yüksek saptandı (sırasıyla p<0.01 ve p<0.001). Sepsinin erken tanısında hematolojik markerlerden biri ile fîbronektin veya haptoglobin 'in birlikte değerlendirilmesi, her bir parametrenin tek başına değerlendirilmesinden daha hassas olduğu görüldü. Sonuç olarak, yenidoğan sepsisinde hematolojik parametreler ile akut faz proteinlerinin birlikte kullanılması geleneksel mikrobiyolojik tanı yöntemlerinden daha hızlı tanı koyma imkanı sağlayacağı görüşüne varıldı.
Abstract
The fibronectin, haptoglobin, ceruloplasmin, and transferrin concentrations were measured in 24 term neonates with culture proven sepsis using a control of 17 normal neonates with similar age, sex and weight. The white blood cell count, and the ration of band to neutrophils were also determined. The fibronectin and haptoglobin concentrations were differed significantly between two groups (p<0.05). In contrast, ceruloplasmin and transferrin levels were similar in infected and noninfected neonates. Total neutrophil count and band/neutrophil ratio in peripheral differential leukocyte count were found significantly elevated in the sepsis group (p<0.01 andp<0.001, respectively). The combination of a hematological marker and fibronectin or haptoglobin seems to be particularly predictive of sepsis compared to each one of the parameters. The combination of hematological and acute phase protein tests may provide a more rapid diagnosis of neonatal sepsis than conventional microbiological methods.
