Özet

Bir donör-hastadan alman kanın daha sonra tekrar hastanın kendisine verilmesi olarak tanımlanan otolog kan transfiizyonunun elektifjinekolojik operasyonlarda uygulanabilirliği araştırıldı. Toplam 248 hastadan, sağlık durumu uygun olan (hematokrit değeri >%35, ağırlık >50 kg ve 18-55 yaşlar arasında) 92'sine (%37) otolog kan transfüzyonu teklif edildi. Bu olgulardan 59'u (%64) çalışma programına alındı. Olgulardan 350-400cc (I ünite) kan alındı ve operasyona kadar 6-15 gün süreyle günde 300 mg ferrous sulfate verildi. Toplam 55 olguda otolog kan transfüzyonu programı tamamlandı. Otolog kan alınması esnasında %7 oranında geçici hipotansiyon ve baş ağrısı gibi yan etkiler görüldü. Başlangıç, preoperatif ve postoperatif ortalama hematokrit değerleri sırasıyla %39.66, %37.18 ve %35.82 bulundu. Olgularımız ortalama 8.25gün sonra operasyona alındı. Alman kanların %29'u tekrar aynı hastalara verildi. Otolog retransfüzyona ilaveten %1.8 oranında homolog kan transfüzyonu yapıldı. Olgular postoperatif ortalama 5.78 günde taburcu edildi. Otolog alman kanların %71 'i diğer acil ve elektif olgularda homolog transfüzyon için kullanıldı. Elektifjinekolojik cerrahi girişimlerde uygulanan otolog kan transfüzyonlan, hastaları alloimmün reaksiyonlardan ve infeksiyöz hastalıklardan korumaktadır. Bu uygulamanın ülkemiz için bir problem olan kan temini konusunda kolaylık sağlayacağını düşünmekteyiz.

Abstract

The effectiveness of autologous blood transfusion which is defined as blood collected from the same patient for retransfusion to himself at a later time was investigated in elective gynecological surgery. Autologous blood donation was offered to 92 healthy women in 248 preoperative cases (37%). For autologous blood donation parameters that we accepted appropriate for health were the haematocrit value >35%, the body weight >50 kg and the range of age 18-55 years. Fifty-nine (64%) eligible patients were enrolled into the study programme. Each patient gave one unit of blood (350-400 ml) and took 300 mg ferrous sulphate daily for 6-15 days before gynecological surgery. A total number of 55 cases completed the autologous blood transfusion programme. The side effects such as transient hypotension and headache related to blood donation were found in 7% of the participants. Initial, preoperative and postoperative mean hematocrit values were 39.66 %, 37.18% and 35.82%, respectively. Patients were operated in 8.25 days after blood donation. During hospitalization, 29% of the cases were retransfused. Additionally 1.8% of the cases necessitated homologous blood transfusion. After the operation, patients were discharged in 5.78 days. The released packed bloods (71 %) were subsequently administered to other emergency and elective cases necessitating transfusion. In elective gynecological surgery, autologous blood transfusions eliminates the risks of infection and alloimmunization. We think that this procedure could also provide a significant facility about'ihe:problem of blood assurance in our country.
