Akut idiyopatik trombositopenik purpuralı olgularımızın retrospektif olarak değerlendirilmesi

Özet

1989-1996 yılları arasında izlenen, yaşlan 1-17 yıl arasında değişen 38 akut idiyopatik trombositopenik purpuralı (ITP) hasta retrospektif olarak incelendi. Deri kanamaları % 89.5, epistaksis % 21.1, gaitada gizli kanama % 13.2, mikroskopik hematuri % 10.5, oral mukoza ve dişeti kanamaları % 7.9 olguda görüldü.Trombosit sayısı 20.000/mm3 'ün üzerinde ve cilt bulguları dışında asemptomatik olan 29 olguya travmadan korunma ve gözlem dışında bir tedavi uygulanmadı. Trombosit sayısı 20.000/mm 'ün üzerinde olup epistaksis ve mukozal kanamaları olan 5 olguya oral prednizolon, 3 haftalık izlemde trombosit sayısı 35.000/mm3 'ün altında olan 4 olgunun 2 'sine intravenöz yüksek doz metil prednizolon, 2 'sine de intravenöz immünoglabulin tedavisi kemik iliği aspirasyonu yapıldıktan sonra başlandı. 6 ay içinde remisyon % 92.1 'e ulaşırken 3 olguda (% 7.9) kronikleşme görüldü. Çalışmanın sonucunda kemik iliği aspirasyonunun sadece ayırıcı tanı gerekenler ve steroid tedavisi verilecek olanlara yapılması gerektiği, trombositopeni ve cilt bulguları dışında semptomu olmayan olgulara steroid veya başka bir ilaç tedavisinin gerekli olmadığı kanısına varıldı.

Abstract

38 children with acute idiopathic thrombocytopenic purpura (ITP) who were followed between the years of 1989-1996 are retrospectively analyzed. Their ages were between 1 to 17 years (mean age was 8.2 years).There was skin bleeding in the great majority of cases (89.5 %), epistaxis (21.1 %), fecal microscopic bleeding (13.2%), microscopic hematuria (10.5 %), oral mucosal and gingival bleeding (7.9 %).The 29 patients with no physical findings except skin bleeding and platelet counts more than 20xl03/mm3 were not given any therapy but they continued observation and prevented from possible traumatic events. The 5 patients with the platelet counts under 20x103/mm3 and have physical findings of epistaxis or mucosal bleeding were undergone to oral prednisolone; 2 of 4 other patients whose platelet counts under 35x103/mm3 after three weeks observation were given high dosage of methyl prednisolone intravenously, and the other 2 were undergone to intravenous immunoglobulin therapy, after having bone marrow aspiration. The remission rate in a six months period was 92.1 %, and the chronicity was seen in three cases (7.9%). As a result of this study, bone marrow aspiration is necessary for only the patients who require differential diagnosis or will take corticosteroid therapy. Corticosteroids or any other drug therapy are unnecessary for the patients with ITP who have not any physical findings except skin bleeding.
