Özet

Yenidoğan yoğun bakım ünitelerinde sık rastlanan bir gastrointestinal sistem hastalığı olan nekrotizan enterokolit (NEK); barsak mukoza yüzeyinde iskemiye neden olabilecek hastalıkları bulunan, düşük doğum ağırlıklı bebeklerde daha sık görülür. Bu çalışmada 1993 ile 1996 yılları arasında Çukurova Üniversitesi Tıp Fakültesi Yenidoğan Yoğun Bakım Ünitesi'nde yatırılarak tedavi edilen 2625 bebeğin dosyası retrospektif olarak incelendi ve nekrotizan enterokolit düşünülen 54 olgunun klinik, laboratuvar, radyolojik bulguları ve risk faktörleri gözden geçirildi. Bu olguların 29'u şüpheli (grup l), 25'i ise belirgin (grup II) nekrotizan enterokolit olgularıydı. Tüm NEK olgularının 48'ini (%89) preterm bebekler oluşturuyordu. Birinci gruptaki bebeklerin ortalama gebelik yaşı 31.7±3.3 (24-40) hafta ve ortalama doğum ağırlığı 1720+570 (774-2930) gram iken ikinci gruptaki bebeklerin ortalama doğum ağırlığı 1700+690 (880-3512) gram idi. Her iki gruptada ortanca tanı yaşı 6. gündü. Birinci gruptaki bebeklerin 16 (%55)'sı hiç beslenmemişken bu sayı ikinci grupta 6 (%24) idi. Ortanca beslenme yaşı birinci grupta 3.5 (1-27) gün iken ikinci grupta ise 5.5 (1-11) gün olarak belirlendi. Her iki grupta en sık rastlanan klinik bulgu abdominal distansiyondu, bunu letarji ve apne takip ediyordu. Birinci grupta 14 (%48), ikinci grupta ise 20 (%80) bebekte trombositopeni mevcuttu. İkinci grupta 18 (%72) bebekte pnömatozis intestinalis, yedi bebekte diyafragma altında serbest hava, bir bebekte de portal vende gaz tespit edildi. Birinci gruptaki 12 (%41) bebeğe sepsis eşlik ediyorken, ikinci gruptaki 18 (%72) bebeğe sepsis eşlik ediyordu. Her iki grubun kan kültürlerinde en sık üretilen mikroorganizma Klebsiella ve Enterobacter türleriydi.

Abstract

Necrotizing enterocolitis (NEC) is the most common serious gastrointestinal disorder encountered in the neonatal intensive care units. Most cases occur in preterm and low-birth-weight infants who often are already compromised by their associated illness. The present study was performed in the Neonatal Intensive Care Unit at Çukurova University and clinical, laboratory and radiologie flndigs of 54 infants who were thought to have necrotizing enterocolitis (NEC) were evaluated retrospectively and the risk factors tried to be assessed. Twenty nine of these were suspected (Grup I), and 25 were confirmed (Grup II) cases of NEC. Fourty eight (89%) of all NEC cases were preterm babies. The mean gestationel age of the babies in the first group was 31.7+3.3 (24-40) weeks, with a mean birth weight of 1720+570 (774-2930) grams; the mean gestational age and birth weight of the babies in the second group was 31.9±3.9 (24-40) weeks, and 1700+690 (880-3512) grams respectively. The mean age at the time of diagnosis was six days in the both groups. Fiften (51%) infants and, 12 (48%) babies in the first group and was six infants (24%) in the second group were never fed. The median age of days in the second group. In both groups, the most frequent finding was abdominaal distansion followed by lethargy and apnea. Thrombocythopenia was present in 14 (48%) patients in group I, and in 20 (80%) patients in group II. Among infants in the second group 18 (72%) had pneumatosis intestinalis, seven had free air under the diaphragm and are had gas in portal vein. NEC was associated with sepsis in twelve (41%) babies in group I and 18 (72%) babies in group II. Klebsiella and Enterobacter species were the most frequently isolated microorganisms from the blood cultures of septic infants in both groups.
