Özet

Bu çalışmada İzmir Dr. Behçet Uz Çocuk Hastanesi Romatoloji Polikliniğimizce izlenen, yaşları 4 ile 15 yıl arasında değişen; l 'i erkek 8 'i kız 9 Sistemik Lupus Eritematozus 'lu hastanın klinik ve laboratuar bulguları, renal biopsi sonuçları ve tedavi yaklaşımları sunulmuştur. Serimizde ortalama hastalık başlangıç yaşı 11.2 yıldır; en sık görülen bulgular ise nefrit (%100), artralji veya artrit (%88.8), konstitüsyonel bulgular (%66.6), deri döküntüsü veya malar ras (%55.5) 'tir. ANA (+) 'liği 100, anti-DNA (+) 'liği %77.7, C3 düşüklüğü %88.8 olarak saptanmıştır.Lupus nefritinin histopatolojik tipi ile renal bulgular arasında bir ilişki olduğu bilinmektedir. Ancak üriner sediment değişikliklerinin olmaması renal patoloji bulunmadığı anlamına da gelmez. Hastalarımızın yedisine renal tutulumu belirlemek ve histopatolojik bulguya göre tedaviyi yönlendirmek amacıyla renal biopsi uygulanmıştır. Sonuçta klinik bulguların derecesinin herzaman renal patoloji ile paralellik göstermediği saptanarak, önemi vurgulanmıştır

Abstract

In this study nine cases (18 girls, 1 boy of ages between 4 and 15 years) of systemic lupus erythematosus diagnosed and followed up at Izmir Dr. Behçet Uz Children's Hospital Rheumatology Department are presented. Clinical, laboratory and renal biopsy findings and the therapy approaches are evaluated. In our series the mean age at the disease onset is 11.2 years; the most common findings are nephritis (100%), articular findings (88.85), constitutional symptoms (66.6%), skin eruptions and malar rash (55.5%). ANA positivity is 100%, anti-DNA positivity is 77.7%, low levels of complement component 3 is 88.8%.It is known that there is a relationship between the histopathologic type of lupus nephritis and the severity of clinical renal findings. On the other hand lack of urinary sediment changes does not rule out renal involvement. We performed renal biopsy to 7 our patients in order to evaluate the renal involvement and make decision about the disease management. In conclusion it is detected and emphasized that the severity of clinical findings is not always parallel to the renal histopathology.
