Özet

Bu retrospektif çalışmada, 4000 gr. ve üzerinde doğan yenidoğanlarda perinatal ve maternal prognozu belirlemek için Dr.Zekai Tahir Burak Kadın Hastanesinde 24.9.1994 - 31.12.1995 tarihleri arasında > 4000 gr doğan 1008 infant ve < 4000 gr. doğan 23800 infantın maternal ve neonatal kayıtları incelendi. Doğum şekilleri, maternal ve perinatal komplikasyonlar saptandı. Kliniğimizde çalışma periodu süresinde gerçekleşen 24808 doğum içinde makrozomi oranı %4.06 olarak gerçekleşti. Makrozomik grupta primer sezaryen oram (%21.3), kontrol grubundan (%15.36) belirgin yüksekti (p<0.05). Makrozomik infantlarda erkek cinsiyet, yüksek doğum travması ve düşük APGAR oranları gözlenirken, yine makrozomi grubunda maternal genital laserasyon oranlarıda kontrol grubuna göre belirgin yüksekti (p<0.005). Sonuç olarak doğum haftası, maternal kilo, multiparite, daha önceki doğumlarda makrozomik bebek öyküsü, erkek cinsiyet fetal makrozomi ile ilişkili bulundu ve oluşabilecek ciddi problemler nedeniyle risk altındaki kadınlar makrozomik infant açısından özellikle taranmalıdırlar.

Abstract

In this retrospective study, maternal and neonatal records of infants weighing > 4000gr (n=1008) and <4000gr (n=23800) were identified between 24.9.1994 and 31.12.1995.In order to describe the neonatal and maternal outcome of macrosomic infats weighing at least 4000 gr, the obstetrical outcome variables of the groups, including the mode of delivery and incidence of maternal and perinatal complications were compared. Among the 24808 deliveries during the study period in our institution, macrosomic infant rate was found as 4.06% . The overall primary cesarean section rate in the macrosomic group was 21.73% and significantly higher than the control group 15.36% (p<0.05) . The macrosomic infants were more often male ,had more birth trauma and lower APGAR scores. Maternal complications of the macrosomia including genital lacerations requiring repair were significantly higher than those of controls (p<0.05) . In conclusion, a number of factors have been associated with fetal macrasomia : gestational age, maternal weight, multiparity, macrosomia in a prior infant and male sex. Because of serious perinatal problems, women at risk should be screened for macrosomia.
