Portal hipertansif gastropatinin endoskopik ve histopatolojik bulgularının karşılaştırılması

AMAÇ: Bu çalışmada, portal hipertansif gastropatinin endoskopik ve histopatolojik bulgularının karşılaştırılması ile siroz etyolojisi, Child sınıflaması ve ösefagus varis evrelerinin korelasyonu araştırıldı. YÖNTEM: Histopatolojik olarak siroz tanısı konmuş ve portal hipertansiyon gelişmiş 20 hasta çalışmaya alındı. Başka nedenlerle gastroskopi endikasyonu konan ve endoskopik bulguları normal olan 20 hasta kontrol grubunu oluşturdu. Hasta grubunda hepatit markerları, Child evrelemesi için byokimyasal parametreler, batın ultrasonografısi, karaciğer iğne biyopsisi, panendoskopi ve mide fundus ve antrum biyopsileri uygulandı. BULGULAR: Child sınıflaması ile portal hipertansif gastropati arasında anlamlı ilişki bulunurken (p<0.005), endoskopik görünüm ile histopatolojik bulgular arasında anlamlı ilişki saptanmadı.SONUÇ: Çalışmamızda Child Pugh evresinin ağırlığı ile portal hipertansif gastropati arasındaki anlamlı ilişki karaciğer yetmezliğindeki mediatörlerin gastropatiye katkıda bulunduğunu düşündürmektedir. Endoskopik görünüm ile histopatolojik bulgular arasında anlamlı ilişki olmaması portal hipertansif gastropatinin ağırlığının saptanmasında ancak biyopsinin anlamlı sonuç verebileceğini düşündürmektedir.
Abstract
OBJECTIVES: In this study, we investigated the correlation of endoscopic and histopathological findings of portal hypertensive gastropathy and their relation with etiology of cirrhosis. Child-Pugh's classification and the severity of the osephageal varisces in cirrhotic patients. METHOD: 20 cirrhotic patients with portal hypertension whose diagnosis were made by liver biopsy, were evaluated and 20 patients whose endoscopy were made for disorders other than cirrhosis were made, biopsies from gastric fundus and antrum were taken in both groups. Liver biopsy was performed in the study group only. RESULTS: We found a significant relationship between Child classitacation and portal hypertensive gastropathy but no correlation between endoscopic and histopathologic findings. CONCLUSION: The significant correlation between Child-Pugh's classification and portal hypertensive gastropathy may be related to the mediators found in liver cirrhosis. Our study showed that histopathological evaluation is the best method for detection of severity of portal hypertensive gastropathy.
