Plazma ve rezidüel gastrik sıvıdaki prostaglandin E2 düzeyi ile nonsteroid antiinflamatuar ilaç kullanımı sonucu oluşan gastropati arasındaki ilişki

AMAÇ: Bu çalışmada; iskelet-kas sistemi ve romatizmal hastalıklarda sıklıkla kullanılan ve dünyada en çok reçete edilen nonsteroid antiinflamatuar ilaçlardan nimesulid, naproksen Na ve asemetazin kullanan hastalarda plazma ve rezidüel gastrik sıvıdaki prostaglandin E2 (PGE2) düzeyi ve bu ilaçların kullanımı ile oluşan gastropati arasındaki ilişkinin incelenmesi amaçlanmıştır. YÖNTEM: Çalışmaya; Mart ve Haziran 1997 tarihleri arasında, ilk endoskopileri normal olan ve ortalama yaşları 21.10±1.29 yıl olan, 100 erkek hasta dahil edilmiştir. Hastalar 4 gruba (Plasebo tb. 2x1, Nimesulid 100 mg tb. 2x1, Naproksen Na 550 mg tb. 2x1, Asemetazin 60 mg tb. 2x1) ayrılarak, tedavi öncesi ve 7 günlük tedavi sonrasında plazma ve rezidüel gastrik sıvıdaki PGE2 değerleri, RIA ile çalışıldı. BULGULAR: Plasebo grubunda ilk ve son plazma PGE2 değerleri, 4.56±1.16 pg/ml ve 4.48±1.09 pg/ml (p>0.05), nimesulid grubunda 5.12±3.00 pg/ml ve 2.16±1.33 pg/ml (p 0.05), nimesulid'de 93.03±46.84 pg/ml ve 75.13±39.71 pg/ml (p<0.005), naproksen Na'da 96.24±57.71 pg/ml ve 57.37±46.59 pg/ml (p<0.0001), asemetazinde 103.86±50.53 pg/ml ve 75.38±47.29 pg/ml'di (p<0.0005). Nimesulid ve naproksen Na, nimesulid ve asemetazin, asemetazin ve naproksen Na gruplarının ilk ve son gastrik PGE2 değerlerinin farkları arasında istatistiksel fark vardı (p<0.0005, p<0.05, p<0.01). Nimesulid, naproksen Na ve asemetazin gruplarında gastrik hasarı gösteren ortalama Lanza skorları sırasıyla 0.64±0.81, 2.68±0.74, .64±1.35'ti. Nimesulid-naproksen Na, nimesulid-asemetazin ve asemetazin-naproksen Na grupları arasında istatistiksel anlamlı fark vardı (p<0.0005, p<0.002, p<0.001). Lanza skorlaması ile gastrik PGE2 değerleri arasında negatif korelasyon mevcuttu (r : 0.215). Plazma PGE2 değerleri ile Lanza skoru, plazma ve gastrik PGE2 değerleri arasında korelasyon saplanmadı. HP (+) ve HP (-) olan hastalarda tedavi öncesi PGE2 değerleri sırasıyla 74.93±57.27 pg/ml ve 93.18±60.00 pg/ml (p<0.0001) olup, HP (+) hastalarda düşük olarak bulundu. PGE2 değerleri ile HP (+)'liği arasında zayıf derecede negatif korelasyon mevcuttu (r : 0.362). SONUÇ: Nimesulid grubunda, gastrik lezyonların sayı ve şiddeti ile PGE2 değerlerindeki azalma, asemetazin ve naproksen Na grubuna oranla daha düşük olarak bulunmuştur. PGE2 değerlerindeki azalma ile gastrik mukozal lezyonların sayı ve şiddeti arasında negatif korelasyon vardır. Plazma PGE2 değerlerindeki azalma ile gastrik mukozal lezyonların sayı ve şiddeti arasında korelasyon bulunamamıştır. Rezidüel gastrik sıvıdaki PGE2 değerleri endoskopik olarak gösterilen mukozal değişikliklerin moniterizasyonunda kullanılabilir.
Abstract
AIM: The aim of this study is to investigate the prostaglandin E2 (PGE2) levels in plasma and residual gastric juice of patients using nonsteroidal antiinflammatory drags, and the resultant gastropathy related to these drugs. MATERIAL AND METHODS: 100 male patients, with normal first visit endoscopies, mean age 21.10±1. 29 years, were enrolled to study between March-June 1997. Patients were divided into 4 groups (Placebo tb. bid, Nimesulide 100 mg tb. bid, Naproxen Na 550 mg tb. bid, Acemetacin 60 mg tb. bid), and PGE2 levels were measured by RIA in plasma and residual gastric juice before treatment and after 7 days of treatment. RESULTS: First and the last plasma PGE2 levels were 4.56±1.16 pg/ml and 4.48±1.09 pg ml in placebo group (p>0.05), 5.12±3.00 pg/ml and 2. 16±l.33 pg/ml in nimesulide group (p>0.05), 4.09±2.42 pg ml and 1.25±0.51 pg ml in naproxen Na group (p>0.05), 4.53±1.11 pg/ml and 1.24±0.69 pg/ml in acemetacin group (p (1.05). There was no statistically significant difference nimesulide, naproxen Na and acemetacin groups. First and the last gastric PGE2 levels were 79.59±34.36 pg/ml and 79±34.72 pg/ml in placebo group (p>0.05), 93.03±46.84 pg/ml and 75.13±39. 71 pg/ ml in nimesulide group (p<0.005), 96.24±57.71 pg/ml and 57.37±46.59 pg/ml in naproxen Na group (p< 0.0001). 103.86 ±50).53 pg/ml and 75.38±47.29 pg/ml in acemetacin group (p<0.0005). There was a statistically significant difference in the first and the last PGE2 levels of nimesulide-naproxen Na, nimesulide-acemetacin and acemetacin-naproksen Na groups (p<0.0005, p<0.05, p<0.01). Lanza score which shows the gastric damage in nimesulide, naproxen Na and acemetacin group was 0.64±0.81, 2.68±0.74, 1.64±1.35, respectively. There was a statistically significant difference between nimesulide-naproxen Na, nimesulide-acemetacin and acemelacin-naproxen Na groups (p <1.0005, p<0.002, p<0.001). There was a weak negative correlation between the Lanza score and gastric PGE2 levels (r : 0.215). There was no correlation between the Lanza score and plasma PGE2 levels and plasma and gastric PGE2 levels. Gastric PGE2 levels of HP pozitive patients (74.93±57.27 pg/ml) were lower than HP negative patients (93.18±60.00 pg/ml) (p<0.0001). There was a weak negative correlation between PGF.2 levels and HP pozitivity (r : 0.362). CONCLUSION: Nimesulide group, the number and severity of gastric lesions related to the reduction in PGE2 levels, were lower than acemetacin and naproxen Na group. There was a negative correlation between the reduction in gastric PGE2 levels and the number and severity of gastric lesions. There was no correlation between the reduction in plasma PGE2 levels and the number and severity of gastric lesions. PGE2 measurement in residual gastric juice may be useful in monitoring gastric mucosal damage and may identify patients who are likely to develop endoscopic mucosal changes.
