Özet
AMAÇ: Bu çalışmada, yüksek risk kriterlerine sahip olan ve pulmoner arter kateterizasyonu uygulanan olgulardan elde edilen hemodinamik ve 0 2 transport parametrelerinin prognostik değerinin araştırılması amaçlanmıştır. GEREÇ VE YÖNTEM: Bu amaçla, çalışma kapsamına alınan 32 olgu prognozlarına göre yaşayan(Grup-1) ve yaşamayan(Grup-2) olmak üzere iki gruba ayrılmış ve her gruptaki olgulardan elde edilen sonuçlar her parametre için ortalamaları alınarak ve birbirleri ile karşılıklı olarak değerlendirilerek prognostik önemleri saptanmaya çalışılmıştır. BULGULAR: Her iki gruptaki olgulardan elde edilen parametreler karşılaştırıldığında, DO 2 VO 2 ve Cl için istatistiksel olarak anlamlı fark saptanmamış, buna karşılık SvO 2 , O 2 ekstraksiyonu ve STA gibi parametreler arasındaki fark ise anlamlı bulunmuştur. Lojistik regresyon analizinde sadece Sv02'nün prediktif değeri olduğu ve tek belirleyici değişken olarak tanımlanabileceği ortaya çıkmıştır. Anova analizinde ise sadece yaşamayan olgularda patolojik dağılım bağımlılığı saptanmıştır. SONUÇ: Bu çalışmada SvO 2 , O 2 ekstraksiyonu ve STA gibi parametrelerde istatistiksel olarak anlamlı fark bulunmuş ve özellikle de SvO 2 'nün tek belirleyici değişken olarak kritik cerrahi hastalarda prognostik amaçla kullanılabileceği sonucuna varılmıştır.
Abstract
PURPOSE: In this study we aimed to evaluate the prognostic values of hemodynamic and 02 transport parameters in the pulmonary artery catheterised high-risk patients. MATERYAL AND METHOD: Thirtytwo cases were grouped into two. according to their prognosis. Group I was consisted from the surviving and Group 2 nonsurviving patients. The results obtained from each group were calculated as means and by compared with each other in order to assess their prognostic values. RESULTS: No statistical significance can be detected between the two groups in terms of DO<sub>2</sub>, V0<sub>2</sub> and Cl but in SvO<sub>2</sub>, O<sub>2</sub> extraction and STA the differences were found to be statistically significant. In the logistic regression analysis the only parameter that have predictive value was found to be SvO<sub>2</sub>, so it was defined to be the principal predictive parameter. In the Anova analysis pathologic supply dependency states were detected only in group 2 patients. CONCLUSION: SvO<sub>2</sub>, O<sub>2</sub> extraction and STA the differences were found to be statistically significant between the two groups and specially SvO<sub>2</sub> can be the main predictive parameter in determination of the prognosis in high-risk patients.
