Ratlarda portal triad oklüzyonunun eşzamanlı kolon anastomozunun iyileşmesi üzerine etkileri

Özet
AMAÇ: Travmatik kolon yaralanmaları sıklıkla karaciğer yaralanması ile birlikte görülür. Bu tip çoklu organ yaralanmalarında öncelikle karaciğerden kaynaklanan kanamanın durdurulması gerekebilir. Hemostazı kolaylaştırmak için yapılacak komplet portal triad oklüzyonu splanik alanda konjesyona neden olur. Bu durumun, aynı seansta yapılacak bir primer kolon anastomozu üzerine muthemel etkileri bir rat modelinde araştırılmıştır. GEREÇ VE YÖNTEM: Çalışmada kullanılan 30 adet Wistar rat üç gruba ayrıldı. Grup A 'da yer alan kontrol grubu ratlarda sadece lcm.lik transvers kolon rezeksiyonunu takiben kolo-kolonik anastomoz yapıldı. Grup B'de ise laparotomiyi takiben portal triad 15 dakika süreyle klempe edildi, daha sonra 30 dakika süreyle reperfüzyona izin verildi ve bundan sonra da transvers kolon rezeksiyonu ve anastomoz yapıldı. Grup C'de ise 30 dakikalık oklüzyon ve 30 dakikalık reperfüzyon sonrasında aynı cerrahi işlem uygulandı. Tüm deneklerde serum ALT, AST, LDH, total bilirubin düzeyleri preoperatif ve postoperatif dönemlerde çalışıldı; rezeke edilen transvers kolon segmenti histopatolojik olarak incelendi ve bu dokudaki süperoksit dismutaz değerleri belirlendi. Ratlar 7.günde sakrifiye edilerek anastomoz patlama basınçları ölçüldü, anastomotik segmentin histopatolojik incelenmesi yapıldı ve doku hidroksiprolin değerleri çalışıldı. BULGULAR: Portal triadın otuz dakika süreyle klempe edildiği Grup C'de %90 oranında erken mortalite gözlendi. Bu grup daha sonraki değerlendirmelere dahil edilemedi. Grup B'deki raflardan iskemi/reperfüzyon süreci sonrasında rezeke edilen transvers kolon segmentlerinde hafif ila orta derecede histolojik değişiklikler kaydedildi. Aynı dokuda yapılan süperoksit dismutaz çalışmasında ise iki grup arasında anlamlı farklılık bulunmadı. İki grubun postoperatif 7.gündeki anastomoz patlama basıncı ölçümleri ve anastomotik doku hidroksiprolin düzeyleri arasında farklılık saptanmadı. Anastomoz iyileşmesinin histopatolojik değerlendirilmesinde de anlamlı düzeyde farklılık gözlenmedi. SONUÇ: Ratlarda yapılan 15 dakikalık portal triad okliizyonunun aynı seansta yapılan kolon anastomozu üzerine zararlı etkisi gözlenmemiştir.
Abstract
AIM: Traumatic colon injuries frequently coexist with liver injury. Stopping the bleeding from hepatic pool may require complete portal triad occlusion. The possible effects of splanchnic congestion secondary to portal venous occMATERIAL AND METHODS: A colo-colonic anastomosis following resection of a 1 cm segment of the transverse colon was done with just a simple manipulation on the liver and portal triad in the group A. In the group B the portal triad was clamped for 15 minutes. Then, the clamp was released for a thirty minutes reperfusion period. Subsequently, the same technique in the control group for colonic resection and anastomosis was applied. A 30-minute ischemia and a 30-minute reperfusion were applied with the same colonic procedure in the group C. Pre-and post-operative levels of serum ALT, AST, LDH and total bilirubine were studied. Tissue superoxide dismutase values of resected transvers colon segments were determined. The rats were sacrificed on day-7. Bursting pressures of anastomoses were measured. Anastomotic segments were examined histopathologically. Anastomotic tissue hydroxyprolene values were studied. RESULTS: Thirty-minute portal triad occlusion resulted in a 90% mortality. Therefore, group C was excluded from the study. Ischemia/reperfusion resulted in histologically proven alterations in the large bowel in the group B: However, colonic tissue superoxide dismutase values showed no significant differences between the groups. On day 7, no differences were recorded in bursting pressures of the anastomoses and the hydroxyprolene levels of the anastomotic tissues of the two groups. Histopathologic evaluation of anastomotic healing process also showed no diffrence between grup A and B. . CONCLUSION: Colonic anastomosis in rats after a 15-minutes portal triad occlusion is safe. lusion on healing of colonic anastomosis were examined using a rat model.
