Serum Beta-Hcg serbest fraksiyonu ve progesteron değerlerinin abortus imminens vakalarının prognozunun prediksiyonunda kullanılması

Özet

AMAÇ: Gebeliğin birinci trimesterinde karşılaşılan problemlerden olan abortus imminens olgularında olayın klinik olarak nasıl sonuçlanacağını kestirebilmek amacıyla potansiyel belirleyicilerden olan serum serbest -hCG ve progesteron değerlerinin tek başına ve birlikte kullanılmasının yararını araştırmak. GEREÇ VE YÖNTEM: 7-10. gebelik haftaları arasında vaginal kanama şikayeti ile Celal Bayar Üniversitesi Kadın Hastalıkları ve Doğum Kliniği'ne ve Manisa Kadın Hastalıkları ve Doğum Hastanesi'ne başvuran ve abortus imminens tanısı alan 29 olguda ve benzer gestasyonel haftadaki 25 sağlıklı kontrol olgusunda serum serbest -hCG ve progesteron düzeyleri araştırıldı. BULGULAR: Abortus imminens grubunda serbest -hCG değerleri 13,45 ± 1,34 mIU/ mL, progesteron değerleri ise 22,56 ± 1,76 ng/ml iken kontrol grubunda serbest -hCG değerleri 22,47+1,06 mIU/ mL, progesteron değerleri ise 31,34 ± 1,67 ng/ml idi. (p<0.0001 ve p<0.01). Serum -hCG serbest fraksiyon düzeyinin gebeliğin devamı konusundaki belirleyiciliği için 20 ng/ml sınır değer kabul edildiğinde sensitivite % 61,5; spesifisite % 42,8; pozitif belirleyici değer (PPV) % 57,8, negatif belirleyici değer (NPV) ise % 75,0 olarak hesaplandı. Progesteron değerleri için ise sınır değer olarak 25 ng/ ml alındığında sensitivite %80,0; spesifisite % 66,6; PPV % 75,0; NPV ise % 72,7 olarak hesaplandı. Her iki değer birlikte değerlendirildiğinde ise sensitivite % 60,0, spesifisite % 80,0, PPV % 56,2, NPV ise % 78,3 olarak hesaplandı. SONUÇ: Abortus imminens olgularında serum serbest -hCG düzeylerinin ölçülmesinin progesteron değerlerinin sağladığı klinik belirleyiciliğe anlamlı bir katkı sağlamadığı gözlendi. Bu olgularda gebeliğin prognozu hakkında daha duyarlı belirleyicilere gereksinim vardır

Abstract

OBJECTIVE: To investigate the value of using serum free--hCG and progesterone measurements as potential clinical predictors in threatened abortion, which is one of the main problems during first trimester of pregnancy. MATERIALS AND METHODS: Twenty-five women admitted to the Department of Obstetrics and Gynecology, University of Celal Bayar with the diagnosis of threatened abortion were included into the study. Serum levels of free -hCG and progesterone were measured and compared with that of 29 healthy pregnant women of similar gestational ages. RESULTS: Free -hCG and progesterone levels were 13,45 ± 1,34 mIU/ mL and 22,56 ± 1,76 ng/ml in study group and 22,47+1,06 mIU/ mL and 31,34 ± 1,67 ng/ml in control group respectively (p<0.0001 and p<0.01). As a prognostic tool using cut-off level of 20 mIU/ml, sensitivity, specificity, positive and negative predictive values of free -hCG were 61,5%, 42,8%, 57,8% and 75,0% respectively. Progesterone level using a cut-off level of 25/ng/ml, had a specificity, positive and negative predictive values of 80,0%, 66,6%, 75,0% and 27,7% respectively. Using both values together had a specificity, positive and negative predictive values of 60,0%, 80,0% and 56,2% respectively. CONCLUSION: We found that measuring serum free -hCG levels did not add any power above measuring progesterone levels in cases with threatened abortion. More sensitive and powerful markers are needed to estimate the survival in these cases
