Premenopoz ve menopozlu olguların demografik biyokimyasal ultrasonografik ve kemik mineral dansitometrik özellikleri

Özet
AMAÇ: Bu çalışmanın amacı, menopoz-osteoporozla ilgili tedavi planlarını hazırlamak için menopoz ve premenopoz dönemindeki olguların demografik, biyokimyasal, ultrasonografik ve kemik mineral dansitometrik parametrelerini değerlendirmektir. GEREÇ VE YÖNTEM: Bu çalışma, Kırıkkale Üniversitesi Tıp Fakültesi Araştırma ve Uygulama Hastanesi Kadın Hastalıkları ve Doğum AD polikliniğine başvuran spontan menopozlu 94, cerrahi menopozlu 20 ve premenopozlu 66 toplam 180 olgu da yapıldı. Tüm hastaların boy ve vücut ağırlıkları ölçüldü.Vücut Kitle İndeksi, Ağırlık (kg) / Boy (m2) formülü ile bulundu. Tüm olguların serum biyokimya (açlık kan şekeri,total kolesterol, trigliserid, yüksek yoğunluklu lipoprotein-kolesterol, düşük yoğunluklu lipoprotein-kolesterol, çok düşük yoğunluklu lipoprotein-kolesterol ve hormon analizi (FSH, LH, E2) yapıldı. Osteoporozu belirlemek için lumbal vertebraların kemik mineral dansitometrik ölçümleri yapıldı. Premenopoz ve spontan menopozlu olguların ultrasonografik endometrium kalınlığı değerlendirildi.Hormon replasmanı yapılacak olgulara mammografi yapıldı. Elde edilen sonuçların istatistiksel analizleri SPSS paket programında Kruskal Vallis varyans analizi ve Mann Whitney U testi ile değerlendirildi. İstatistiksel olarak p değeri 0.05'den küçük ise anlamlı kabul edildi. BULGULAR: Premenopoz grubunda yer alan olguların yaş ortalaması 45.9±3.9, cerrahi menopoz gubunda 48.9±5.2 ve spontan menopoz grubunda ise 49.07±6.3 olarak saptandı.Gruplar arasında yaş bakımından fark saptanmadı (p>0.05). Premenopoz grubunda vücut kitle indeksinin 32.6±4.4 kg/m2, cerrahi menopoz grubunda 32.3±6.6 kg/m2 ve spontan menopoz grubunda 30.5±6.0 kg/m2 olduğu gruplar istatistiksel karşılaştırmalı değerlendirildiğinde spontan menopoz grubunun diğer gruplara göre anlamlı farklılık gösterdiği belirlendi (p 0.005). Buna karşın serum E2 düzeyi premenopozal olgularda menopozal olgulara göre anlamlı yükseklik gösteriyordu (p 0.05). SONUÇ: Cerrahi ve spontan menopozlu olguların; relatif kemik fraktür riskinin düşük düzeylerde kalmasını sağlamak ve yaşam kalitesini yükseltmek için hormon replasmanına karar vermede yukarıda sıralanan tüm analizlerden yararlanılmasının uygun olduğu kanısına varıldı.
Abstract
OBJECTIVE: The aim of this study was to evaluate demographic, biochemical, hormonal and bone mineral dansitometric parameters of premenopausal and menopausal women in order to prepare the theuropeutical approach about menopause and osteoporosis. MATERIALS AND METHODS: Ninety-four women in spontaneous menopause, 20 women in surgery-induced menopause and 66 women in premenopausal period were enrolled in the study. The weight and height of all patients were measured, and body mass index was calculated.Serum biochemical analysis including serum glucose level, total chollesterol, triglyceride, high density lipoprotein-chollesterol, low density lipoprotein-chollesterol, and very low density lipoprotein-chollesterol, hormonal analysis (FSH, LH, E2) were peformed on all patients. Bone mineral dansitometry was applied in order to evaluate osteoporosis. Endometrium thickness was evaluated by using ultrasonography in premenopausal and spontaneous menopausal women.Mammography was performed on patients who would receive hormonal replacement therapy. Statistical analysis was performed by using Kruskall Vallis variant analysis and Mann Whitney U tests. The difference was accepted as significant if p value were less than 0.05. RESULTS: The average age of the patients were 45.9±3.9 in premenopausal group, 48.9±5.2 in surgery-induced menopausal group, 49.07±6.3 in spontaneous menopausal women. There was no statistically significant difference among the groups (p>0.05).Body mass indexes of these groups were 32.6±4.4kg/m2, 32.3 ±6.6kg/m2, 30.5±6.0kg/m2 respectively.There was a statistically significant alteration of body mass index in spontaneous menopause group(p<0.05).Moreover, there was no statistically significant difference among three groups in terms of serum biochemical parameters. While FSH levels showed statistically significant increase in menopausal women (p<0.05), serum LH levels showed slight increase the same group (p>0.05). On the contrary, serum E2 levels were statistically significant higher in premenopausal women than menopausal women (p>0.05) Ultrasonograghic endometrium thickness was significantly different in spontaneous menopausal women than menopausal women (p>0.05). In the evaluattion of bone mineral dansitometry, bone density was significantly higher in premenopausal women than the others (p>0.05). CONCLUSION: We concluded that all investigations mentioned above should be performed for decrease of the risk of bone fracture and to increase the life quality in women with spontaneous or surgery-induced women. The risk of bone fractures is relatively increased in women with spontaneous and surgical menopause. To decrease this risk, hormonal replacement, exercise and use of alendronate sodium seems convenient.
