Özet

AMAÇ: Bu çalışmada respiratuar distress sendromunda (RDS) optik dansitometre ile amniotik sıvı insülin ölçümünün yeri araştırıldı. GEREÇ VE YÖNTEM: Özellikle prematüre infantlarda perinatal morbidite ve mortalitenin en önemli sebebi olan RDS fetal pulmoner sürfaktan eksikliği sonucu oluştuğundan, çalışmamızda fetal akciğer matürasyon tespitinde en fazla kullanılan testlerden biri olan optik dansitometrede elde edilen değerlere amniotik sıvı prolaktin indeksi değerleri karşılaştırıldı. Akciğer matürasyonu tayini için amniosentez yapılan 56 hasta çalışmaya alındı. BULGULAR:Çalışmaya dahil edilen 41 hastanın 35'i preterm olarak değerlendirildi. Bunların 23'ünde (%65), RDS gelişti. Optik dansitometri incelemesinde cut off değer, 0.150; prolaktin indeksinde ise 0.045 olarak alındı. Bunların altındaki değerler, immatür kabul edildi. Prolaktin indeksi 0.045'in altında olan vakalarda sensitivite, spesifisite ve prediktivite değerlerinin optik dansitometre sonuçlarından belirgin olarak daha fazla olduğu görüldü. İstatistiksel analizler "T testi" ile yapıldı. SONUÇ: Fetal akciğer matürasyon testinde prolaktin indeksi güvenirliliğinin yüksek olduğu sonucuna varıldı.

Abstract

OBJECTIVE: study, the measurement of amniotic fluid prolactin and optic densitometry levels for respiratory distress syndrome (RDS) was assessed. MATERIAL AND METHODS: Especially in premature infants, RDS in the most important reason for perinatal morbidity and mortality and results from fetal pulmonary surfactan deficiency. In our study, the values obtained from optic densitometry, which is the most frequently used test for fetal pulmonary maturation; and the values of amniotic fluid prolactin measurements were compared. Fifty-six patients were included to the study. RESULTS: Thirty-five of 41 cases were evaluated as preterm and 23 of them (65%) developed RDS. Cut off levels were: 0.150, for optic dansitometry, and 0.045, for prolactin index. Under these values were accepted as immature pulmonary maturation. In cases which prolactin index value was under 0.045; the sensitivity, spesificity and predictivity rates were significantly higher than the results of this finding. "T test" was used for the statistical analysis. CONCLUSION: It was concluded that the reliabilty of prolactin index among the fetal lung maturity tests was considerably significant.
