Özet
AMAÇ: Ocak 1993-Ağustos 1997 tarihleri arasında Dr. Samı Ulus Çocuk. Hastanesi Nöroloji Kliniğinde Duchenne Müsküler Distrofisi tanısı alan hastaların profilini çıkartmak. YÖNTEM: 13 ay-14 ya f arası 38 hasta retrospektif olarak hastane kayıtları esas alınarak incelendi. Bulgular: Hastaların %84'ü 6-11 yaş grubundadır. En küçük hasta 13 aylık olup, rutin tetkikler sırasında alanın aminotransferaz (ALT), aspartat aminotransferaz (AST) değerlerinin yüksek bulunması üzerine yapılan araştırma sonucu lam almıştır. En sık tanımlanan başlangıç belirtileri yürüme bozukluğu (%95), merdiven çıkmada zorlanma (%68), yerden kalkmada güçlüktür (%47). Semptomların başlangıç yaşı hastaların %35'inde 2 yaşından önce, hemen hepsinde 8 yaşından öncedir. İlk başvuruda hastaların %92'sinde gastroknemius kası hipertrofisi, %79'unda gowers belirtisi. %31.5'inde lomber lordozda artış; tüm kasları atrofik olan bir hasta dışında, hastaların hepsinde kreatin kinaz (CK), %72 'sinde AST ve %92'sinde de ALT normal değerlerden yüksek belirlendi. DNA analizi yapılabilen 13 hastanın 6'sında (%46) delesyon gösterilebilmiştir. Bunların 4'ü santral (üç hastada ekson 45, bir hastada 47-51), ikisi proksimaldir (bir hastada 13-17, bir hastada 6-43). Hastaların %79'unda kardiyolojik yönden patoloji saplanmadı. Seki: hastada (%21) mitral valv prolapsusu, %73.6'sında I.Q. 70'in altında belirlendi. Yürüme kaybı olmayan 16 hastaya gün aşırı tek doz, 20 hastaya ise her ayın ilk 10 günü, tek doz 0.75 mg/kg/gün prednizolon verilmiştir. Birinci grupta 4 (%25), ikinci grupta 3 (%l5) hastanın tedaviden yarar gördüğü belirlenmiştir. SONUÇ: Onsekiz aydan sonra yürüyemeyen, yürümesinde bozulduk olan, yerden kalkamayan, baldır kaslarında hipertrofi belirlenen hastalarda DMD düşünülmelidir.
Abstract
PURPOSE: To demonstrate the profile of Turkish patients with DMD admitted to our hospital. Methods: Thirty eight patients with Duchenne Muscular Dystrophy (DMD), aged from 13 months to 14 years, were retrospectively studied at the department of neurology in Dr Sami Ulus Children 's Hospital between January 1993 and August 1997. RESULTS: Eighty four percent of them were in a group of 6-11 years old. The youngest patient was 13 months old and diagnosed by further analyses necessitated with these high levels of ALT, AST during routine tests. The most frequent initial symptoms were clumsy walking (95%), difficulty in climbing stairs (68%) and rising from the prone position (47%). Age at the beginning of symptoms was under 2 years old in 35 % of patients and almost all of them were under 8 years old. We determined hypertrophy of gastrocnemius muscle in 92%, gowers sign in 79%, increased lumbar lordosis in 31.5%: increasing in creatine kinase (CK.) in all of them excluding one who had atrophy in whole muscles and AST in 72% and ALT in 92% in their first admission. It could be showed deletion in 6 (46%) of 13 patients done DNA analysis. Four of them were central (45 exon in 3, 47-51 exon in one), and two were proximal (13-17 exon in one, 6-43 exon in one). There was no evidence about cardiac pathology in 79% of patients. It was determined prolapsing mitral valv in eigth (21%) and under 70 IQ in 73.6% of patients. Prednisolone, 0.75 mg/kg/day dose, was given to 16 patients who were walking, one dose every other day and 20 in the first ten days in every month. Patients, 4 (25%) in the first group and 3 (15%) in the second group, have benefited from treatment. CONCLUSION: Pediatricians have the responsibility to think of DM D in patients who have clumsy walking, hypertrophy in calf muscles and that can still not walk after 18 months and can not rise from the floor.
