Kupperman İndeksi postmenopozal semptomların değerlendirilmesi için uygun bir skor mudur?

Özet
AMAÇ: Hormon ve Estrojen Replasman Tedavisinin postmenopozal dönemde görülen bulguları ne kadar düzelttiğine dair pek çok değerlendirme sistem ve skoru geliştirilmiştir. Bunların en bilineni Kupperman İndeksi'dir. Biz bu çalışmamızda ERT ile Kupperman İndeksi'nin ve komponentlerinin ne kadar düzeldiğini saptayarak; Hormon Replasman Tedavisini değerlendirmede ne derece etkin olduğunu sorguladık GEREÇ VE YÖNTEM: Hastanemiz Menopoz polikliniğine başvuran ve tedaviye yeni başlayan 105 hastaya üç ayrı protokolde (konjuge equine estrojen, Mikronize esradiol ve transdermal estrojen) ERT verilerek prospektif olarak takip edilmiştir. On bir hasta çeşitli nedenlerle tedaviyi bırakmış, diğer 94 hastaya tedavi öncesi ve sonrası Kupperman indeks skorlaması uygulanıp, karşılaştırılmıştır. BULGULAR: KI skorlamasında yer alan ateş asması, karıncalanma, parestezi, uykusuzluk, çarpıntı, unutkanlık, sinirlilik ve baş ağrısı semptomlarının HRT ile anlamlı derecede düzeldiğini(p<0.05); depresyon, yorgunluk, atralji/myalji ve vertigo şikayetlerinde de değişiklik olmadığını saptadık (p<0.05). Ayrıca tedavi sonrası bakılan estradiol düzeyleri bu grupta anlamlı derecede farklı iken;KI skorları arasında anlamlı fark yoktu (p<0.01 ). SONUÇ: KI skorlaması postmenopozal semptomların değerlendirilmesinde eksik bir skorlama sistemidir
Abstract
OBJECTIVE: Quantifying systems like the Kupperman scale have been developed to measure the effect of hormone replacement therapy on the symptoms encountered during this time. We investigated whether or not the KI is suitable for evaluating postmenopausal symptoms of patients who are given ERT. MATERIAL AND METHODS: A total of 105 surgical menopause patients patients,who attend our outpatient menopause clinic, were recruited and assigned to one of three different ERT regimens (Conjugated equine estrogen, micronised estradiol or transdermal ERT). The patients were subsequently followed for three months. Eleven patients discontinued the treatment regimes. The Kupperman scale was applied before and after ERT for the remaining 94 patients. RESULTS: Hot flushes, formication, parasthesias, insomnia, palpitations, nervousness and headache components of the Kupperman scale improved significantly (p<0.05) following HRT, whereas depressive mood, fatigue, arthralgia and vertigo remained unchanged. During a three months follow up patients, blood estradiol levels were also measured and differences between the HRT regimens were evaluated. Post-HRT estradiol levels were found to be significantly higher than pre-treatment levels (p<0.01 ). CONCLUSION: The KI scale is an imperfect system in evaluating postmenopausal symptoms.
