Özet
Optik nevrit primer varicella enfeksiyonunun nadir bir komplikasyonudur. Literatürde bugüne kadar 12 vaka bildirilmiştir. 5 yaşındaki kız hasta bir haftadır her iki gözünde oluşan ve giderek artan görme kaybı nedeni ile başvurdu. Yakınmalarının başlamasından beş gün önce vücudunda suçiçeği veziküllerinin oluştuğu öğrenildi. Fizik muayenesinde kurutlanmış döküntüler, her iki optik diskte ödem ve hiperemi, afferent pupiller defekt mevcuttu. Bunun dışında nörolojik muayene normaldi. Primer varicella enfeksiyonuna bağlı optik nevrit düşünüldü. İntravenöz metilprednizolon 5 gün, takiben oral prednizolon tedavisi 1 hafta verildi. Tedavinin 4. gününden itibaren görmesi düzelmeye başlayan hastanın 6. ayda görmesi tamamen normale döndü. 18 aylık takibinde yeni bir optik nevrit atağı ya da nörolojik septom gözlenmedi.
Abstract
Optic neuritis is an extremely rare comlication of chickenpox. Twelve cases of optic neuritis associated with chickenpox have been reported in the literature. A five years old female presented with a week history of progressive blurring vision of her eyes. Five days after the onset of visual blurring, she developed the typical vesicular eruption of varicella. On examination she had scabbed rashs and bilateral swelling and hyperemia of optic disks. Absence of bilateral afferent pupillary reflex were detected.The rest of the neurological examination was normal. The diagnosis of acut varicella infection with bilateral optic neuritis was made. Methylprednisolone (5 days, i.v.) followed by oral prednisolone (7 days) was administred to patient. Although, it was still impraired, four days after the initial treatment her visual acuity returned to normal. Her vision had completely restored after 6 months. She has not had any further attacks of optic neuritis or other neurological symptoms 18 months after her admission
