Özet
Konjenital diyafragmatik hernilerin çoğunluğunu Bochdalek hernileri oluşturur. Morgagni hernilerinin oranı %5 olarak bildirilmektedir. Genellikle bu hastalar doğumdan sonra asemptomatiktir ve yetişkin döneme kadar tanı konulamayabilir. 63 yaşındaki kadın hasta nefes darlığı, dudaklarda, dilde ve parmak uçlarında morarma şikayeti ile başvurdu. Hastanın oskültasyonda sağ bazalde solunum seslerinde azalma haricinde patoloji mevcut değildi. Kan gazı tetkikinde ise fizik muayene ile uyumlu olarak ileri derecede arteriyel hipoksisi mevcut idi. Posteroanterior akciğer grafisinde sağ alt zonda infiltrasyon mevcuttu. Bilgisayarlı toraks tomografisinde sağ hemitoraksta hiler seviyenin distalinde anteriorda orta mediastene kadar yükselen adipoz doku dansitesinde kitle tesbit edildi. Tanımlanan doku alanının abdominal boşluktan toraksa hernie olan adipoz dokuya ait olduğu düşünüldü ve hastanın kliniği ile birlikte değerlendirildiğinde Morgagni hernisi tanısı konuldu. Hastaya laparoskopik mesh herniraf i operasyonu uygulandı. Operasyon sonrası hastanın kan gazları belirgin olarak düzeldi ve klinik olarak siyanozu geçti. Erişkin dönemde siyanoz ve dispne ile gelen hastalarda ayırıcı tanıda Morgagni hernisinin akla gelmesini düşünmekteyiz. Morgagni hernisinin tedavisinde laparoskopik yöntemin kullanılması nedeni ile olguyu sunuyoruz.
Abstract
Bochdalek hernias constitute the majority of congenital diaphragm herrnias. Among these Morgagni hernias account for the 5 % of these. These patients are generally asymptomatic after birth, and the diagnosis may be delayed until adulthood. A 63 years old female patient applied with the complaints of shortness of breath, cyanosis of the lips, tongue and fingertips. On physical examination breath sounds were decreased on right basal hemithorax, and cyanosis of the lips, tongue and fingertips were noted. Arterial blood gases yielded severe hypoxia appropriate to the physical examination. There has been observed an infiltration in the basal zone of the right lung on X-ray. Computer tomographic examination revealed the herrniation of an adipose tissue from abdominal cavity to thorax, and the diagnosis of Morgagni hernia has been established. A laparoscopic mesh herniorrhaphy operation has been performed. Arterial blood gases normalized and clinical cyanosis disappeared after the surgery. Here, we presented an adul t case of Morgagni hernia treated with endoscopic mesh herniorrhaphy. We suggest that Morgagni hernia must be kept in mind in the differential diagnosis of adult patients with cyanosis and dispnea
