Özet

AMAÇ: Osteoporoz postmenopozal kadınlarda morbidite ve mortalitenin önemli bir sebebidir. Osteoporoz önlenebilir bir hastalık olduğu için gelişip gelişmeyeceğini tahmin etmek önemlidir. Postmenopozal osteoporozun prediktörlerinden biri olarak dehidroepiandrosteron sülfat (DHEAS) gösterilmektedir. DHEA-S'nın osteoporozlu kadınlarla osteoporozu olmayan kadınlar arasında farklı olup olmadığı halen tartışmalıdır. Bu çalışmanın amacı postmenopozal kadınlarda osteoporoz ile DHEA-S düzeyi arasında ilişki olup olmadığını araştırmaktır. GEREÇ VE YÖNTEM: Çalışmamıza postmenopozal osteoporozu olan 33 kadın alındı. Yaş ve beden kitle endeksleri (BKİ) uyumlu, osteoporozu olmayan 29 kadın kontrol grubunu oluşturdu. Detaylı fizik muayene yapıldıktan sonra DHEA-S düzeylerinin ölçümü için sabah açlık venöz kan örneği alındı. Elde edilen verilerin analizinde Mann Whitney U testi ve Spearman yöntemi ile çift yönlü korelasyon kullanıldı. BULGULAR: Lomber spinal kemik yoğunluğu osteoporotik hastalarda kontrol grubundan daha düşük idi (sırasıyla 0.672±0.58 ve 1.014±0.73, p 0.05). Hasta grubunda kemik mineral yoğunluğu ile vücut ağırlığı (r=0.58; p<0.001) ve BKİ (r=0.40; p=0.019) arasında orta derecede korelasyon saptanırken, kontrol grubunda böyle bir ilişki bulunmadı. SONUÇ: Çalışmamız osteoporozu olan ve olmayan postmenopozal kadınlarda DHEA-S seviyelerinde fark olmadığını göstermiştir. Önceki çalışmalara uygun olarak kemik mineral yoğunluğu ile BKİ arasındaki pozitif ilişkinin artmış olması vücut ağırlığının osteoporoz gelişimindeki önleyici rolünü göstermektedir.

Abstract

AIM: Osteoporosis is one of the most important causes of morbidity and mortality in postmenopausal women. Because osteoporosis is preventable it is important to predict the development of osteoporosis. One of the hypothesized predictors of postmenopausal osteoporosis is dehydroepiandrosterone sulfate (DHEA-S). Whether DHEA-S levels are different in osteoporotic women than those without osteoporosis is still controversial. The aim of this study was to investigate whether DHEA-S levels are related to osteoporosis in postmenopausal women. MATERIALS AND METHODS: Thirty-three women with postmenopausal osteoporosis are involved in the study. Twenty-nine age and body mass index (BMI) matched women without osteoporosis formed the control group. After detailed physical examination, morning fasting blood samples are obtained from each subject for the measurement of DHEA-S levels. RESULTS: Lumbar spinal bone mineral density was significantly lower in the osteoporotic patients (0.672±0.58 vs. 1.014±0.73, respectively; p<0.001). The difference of DHEA-S levels between the osteoporotic patients and the control group was not statistically significant (76.0±53.7 vs. 92.0±65.2, respectively, p>0.05). Bone mineral density was significantly related to weight (r=0.58; p <0.001) and BMI (r=0.40; p=0.019) in the patient group, but not in the control group. DHEA-S showed no significant correlation. CONCLUSION: Our results show that DHEA-S levels are not different in postmenopausal women with and without osteoporosis. The significant relation between BMI and bone mineral density reflects the preventive role of high body weight against osteoporosis, compatible with the previous reports.
