Postmenopozal kadınlarda östrojen ile birlikte didrogesteron kullanımının endometrium histolojisi üzerine olan etkilerinin endometrial biyopsi ile değerlendirilmesi

Özet

AMAÇ: İki değişik östrojen replasman tedavi modalitesinde, kontinü olarak verilen düşük doz didrogesteronun endometrium üzerindeki karşılayıcı etkisinin histolojik olarak değerlendirilmesi amaçlanmıştır. GEREÇ VE YÖNTEM: Daha önce bir hormon tedavisi almamış 75 postmenopozal hasta çalışmaya dahil edildi. 25 olgudan oluşan ilk gruba 6 ay süreyle sürekli olarak oral yolla 0,625 mg/gün konjuge ekin östrojen ve 5 mg/gün didrogesteron verildi. 30 kişiden oluşan ikinci gruba transdermal yolla sürekli olarak 17ß mikronize östradiol ve 5 mg/gün didrogesteron verildi. Kontrol grubu olarak seçilen 3. grup ise postmenopozal dönemde olan son 6 ayda östrojen replasman tedavisi almamış 20 olgudan oluşmaktaydı. 3. ve 6. aylarda karman aspirasyon yöntemiyle elde edilen materyal histolojik olarak incelendi. BULGULAR: 6. ayın sonunda 1.ve 2.gruplardan, 18 olguda yetersiz materyal (%32.7) elde edilirken, 20 olguda atrofik endometrium (%36.4), 8 olguda kronik endometrit (%14.75), 6 olguda proliferatif endometrium (%10.9), 1 olguda ise basit endometrial hiperplazi saptandı (%1.8). Hormon replasman tedavisi verilmeyen kontrol grubuna 6.ayda yapılan endometrial örnekleme sonuçlarına göre 6 olguda yetersiz materyal (%30), 3 olguda atrofik endometrium (%15), 5 olguda kronik endometrit (%25) 3 olguda proliferatif endometrium (%15), 1 olguda da basit endometrial hiperplazi tespit edilmiştir (%5). Her üç çalışma grubundan elde edilen endometrial örnekler karşılaştırıldığında elde edilen sonuçlar arasında istatistiksel olarak (Ki-kare testi) anlamlı bir farklılık bulunmadı (p>0,05). SONUÇ: Hormon replasman tedavisi protokollerinde çok sıklıkla kullandığımız medroksiprogesteron asetatın yan etkilerinin nispeten fazla olmasından dolayı daha kolay tolere edilebilen bir progestogen olan didrogesteronun düşük dozda (5 mg/gün), sürekli verildiğinde gerek oral gerekse transdermal yolla verilen östrojenlerin endometriumdaki istenmeyen etkilerini karşılar.

Abstract

AIM: To assess the opposing effect of low dose continious didrogesterone supplementation in two different estrogen replacement therapy. MATERIALS AND METHODS: This study consists of 75 postmenopausal women. 25 of them were given continious conjugated equine estrogen (0,625 mg/day); 30 of them were given transdermal estrogen and didrogesterone (5 mg/day) wre given to the both groupes. 20 cases who had not been using HRT for at least six months boilt up the control group. On the third and sixth months, endometrial biopsy was performed for the histopathologic diagnosis. RESULTS: In the study group, in 18 of the cases inadequate specimen, in 20 cases atrophic endometrium, in 8 cases chonic endometritis, in 6 cases proliferative endometrium and in 1 case simple endometrial hyperplasia were found. The findings for the control group were nearly the same, and no statistically significant difference was found between all these groupes. CONCLUSION: Didrogesteron at a 5 mg/day low dose may succesfully oppose the undesired sideeffects of both orally or transdermally given estrogen, and is reliable alternative to medroxyprogesteroneacetate.
