Özet

AMAÇ: Postoperatif erken dönemde oral beslenmeye başlamanın postoperatif komplikasyonlar ve hastaneden taburcu edilme zamanı üzerine etkisini araştırmak. GEREÇ VE YÖNTEM: Prospektif olarak planlanmış çalışmaya elektif koşullarda sezaryen operasyonu yapılan 287 hasta dahil edildi. Medikal bir problemi olan hastalar çalışma dışı tutuldu. Çalışma grubunda yer alan 148 hastaya operasyon sonrası barsak sesleri dinlenmeksizin 6. saatte oral gıda başlanırken, kontrol grubunu oluşturan 139 hastaya ise geleneksel şekilde oral gıda rejimi başlandı. Her iki gruptaki hastalar sosyodemografik özellikleri açısından değerlendirildikten sonra intraoperatif ve postoperatif komplikasyonlar ve hastanede kalış süreleri açısından karşılaştırıldı. Tüm hastalar günlük barsak sesi, gaz çıkışı, bulantı, kusma, abdominal gerginlik, oral beslenmeyi tolere edebilme ve postoperatif komplikasyonlar (febril morbidite, kesi yeri enfeksiyonu, ileus) açısından karşılaştırıldı. BULGULAR: Her iki grup; yaş, gravida, parite, abortus, tahmini gebelik haftası ve doğum ağırlığı açısından benzerdi. Erken oral gıda başlanan grupta hastanede kalış süresinin daha kısa olduğu gözlense de, istatistiksel olarak anlamlı değildi (3.1±0.86'ya karşın 3.56±0.64, p>0.05). Febril morbidite ve kesi yeri enfeksiyonu gibi komplikasyonlar açısından iki grup arasında anlamlı farklılık saptanmazken (p>0.05), iki grupta yer alan hastaların hiçbirinde ileus gelişmedi. İstatistiksel olarak anlamlı olmasa dahi çalışma grubunda daha fazla sayıda hastanın ilk 24 saatte gaz çıkardığı gözlendi. SONUÇ: Erken oral gıdaya başlamak postoperatif ve gastrointestinal morbiditeyi olumsuz yönde etkilememektedir.

Abstract

OBJECTIVE: To evaluate the effect of postoperative early feeding on postoperative complications and on patient discharge. MATERIAL AND METHOD: Prospectively designed study included 287 patients who operated on an elective condition excluding patients having any medical problem. Patients in the study group were allocated to take oral feeding on the 6th hour without auscultation, whereas those 139 patients in the control group were began traditional postoperative oral feeding. Both of the groups were evaluated in respect to socio-demographic features, and compared in regard to intraoperative and postoperative complications and in regard to discharge days. All of the patients were compared in respect to commencement of bowel movements, time of pass of flatus, nausea, vomiting, abdominal distention and postoperative complications (febrile morbidity, wound infection, ileus). RESULTS: Both of the groups did not differ in terms of age, gravida, parity, mean number of abortions, gestational age and birth weight. Patients in the study group had a shorter hospital stay, but not statistically significant (3.1±0.86 v.s. 3.56±0.64, p>0.05). The incidence of febrile morbidity and wound infection were same between the groups and ileus was not recorded in the either group. More patients in the study group passed flatus during the first 24 hours even it was not statistically significant. CONCLUSION: Beginning early feeding after cesarean section do not adversely affect postoperative and gastrointestinal morbidity.
