Özet
AMAÇ: Hemanjiomlar karaciğerin en sık izlenen benign lezyonlarıdır. Artmış serum estrojen ve dokulardaki yüksek estrojen reseptör düzeyleri hemanjiom, estrojen ilişkisinin varlığını ortaya koymaktadır. Ancak literatürde bugüne kadar postmenopozal estrojen tedavisinin karaciğer hemanjiomları üzerine olan etkilerini gösteren çalışma yoktur. Bu çalışmada, postmenopozal iki yıl süreyle oral estrojen replasman tedavisi alan kadınlarda tedavinin, hemanjiomların boyutları üzerindeki etkilerini değerlendirmeyi amaçlanmıştır. GEREÇ VE YÖNTEM: En az 1 yıldır adet görmeyen FSH>40 IU/ml postmenopozal 25 hastaya yapılan rutin abdominal ultrasonografide şüphelenilen hemanjiomlar dinamik kompitürize tomografi ile kesin tanı almışlardı. Karaciğer fonksiyon testleri normal olan hastalara iki yıl süreyle oral estrojen replasman tedavisi (0.625 mg/gün konjuge estrojen+2.5 mg/gün medroksi progesteron asetat) başlandı. Takibde gelen 23 hasta aynı doktor tarafından 6 aylık kontrollerle değerlendirildi. BULGULAR: Altı aylık takipler sonrasında 23 hastanın 6 sında hemanjiomlarda 1 mm lik büyüme saptandı. SONUÇ: Altı aylık takipte hemanjiomlar HRT ile büyüme göstermemiştir.
Abstract
OBJECTIVE: Hemangiomas are the most common benign hepatic neoplasms. Increased serum estrogens and tissue estrogen receptors suggest that there is an association between estrogen and hemangiomas. Whereas, in the literature, there is no study which implies to the effect of postmenoposal oral estrogen therapy on liver hemangiomas. The aim of study was to evaluate the effects of postmenoposal estrogen therapy on liver hemangiomas throughout 2 years. MATERIAL AND METHOD: 25 Postmenoposal women being at least 1-year amenorrheic and their FSH level over 40 IU/ml, have been examined routine abdominal ultrasound which has suspected hemangiomas that was certainly diagnosed by dynamic computerized tomography. Oral estrogen therapy (0.625 mg/day conjugated estrogen and 2.5 mg/day medroxyprogesteronacetate) has been initiated to the patients being normal liver function. 23 patients followed-up and have been examined by the same doctor during 6-month controls. RESULT: After the 6-month follow-up, 6 of 23 patients's hemangiomas been grown 1 mm were detected. CONCLUSION: During sixth-month HRT did not change hemangiomas in size.
