Özet

Amaç: Cerrahi gastrostomi uygulanan hastalarda saptanan komplikasyonları, hastaların sağkalım sürelerini ve gastrostomi için uygun hasta gruplarını belirlemek. Gereç ve Yöntem: Çalışmaya 1998-2001 yılları arasında cerrahi gastrostomi uygulanan hastalar alındı. Hastaların yaşı, cinsiyeti, gastrostomi endikasyonları ve tipi, gastrostomi sonrası oluşan komplikasyonlar ve sağkalım süreleri kaydedildi ve genel durumları APACHE-II skorlaması kullanılarak değerlendirildi. APACHE-II skorları ile sağkalım süreleri arasındaki ilişki istatistiksel olarak Fisher'in tam ki-kare testi ile değerlendirildi. Bulgular: Çalışmaya 40 hasta dahil edildi. Hastalar APACHE-II skorlarına göre gruplandığında, 15 hasta 0-5 arası ve 25 hasta >5 skora sahipti. Tüm grupta 30 günlük mortalite %30 iken APACHE-II skoru 0-5 arası olanlarda %6.7 ve >5 olanlarda %44 idi. APACHE-II skoru >5 olan hastalarda 30 günlük mortalite istatistiksel olarak anlamlı şekilde daha yüksekti (p=0.015). Sonuç: Cerrahi gastrostomi endikasyonu konulurken bu girişimden yarar sağlayacak hastaların seçilebilmesi için APACHE-II skorlaması kullanılabilir. APACHE-II skoru >5 olan hastalarda genellikle 30 günden daha kısa bir yaşam beklentisi olduğundan cerrahi gastrostomi seçici olarak uygulanmalıdır.

Abstract

Objective: To determine postoperative complications, patients' survival and appropriate patient groups for surgical gastrostomy. Material and Methods: Patients treated with open surgical gastrostomy between 1998-2001 were included in the study. Patients' age and gender, indications for and type of gastrostomy, postoperative complications and survival times were recorded and patients were evaluated using APACHE-II scoring system. Results: 40 patients were included in the study. When patients were grouped according to APACHE-II scoring system, the number of patients with scores 0-5 and >5 were 15 and 25 respectively. 30-day mortality rate was 6.7% and 44% for patients with scores between 0-5 and >5 respectively, while it was found as 30% for the entire group. 30-day mortality rate was statistically significantly higher in patients with APACHE-II scores >5 (p=0.015). Conclusion: APACHE-II scoring system could be used during patient selection for open surgical gastrostomy. Since 30-day life expectancy is lower for patients with APACHE-II scores >5, open surgical gastrostomy could be performed selectively for these patients.
