Spinal anestezide prilokainle birlikte uygulanan intravenöz efedrin ve intratekal epinefrinin karşılaştırılması

Özet

Amaç: Bu çalışmada alt ekstremite cerrahisinde, spinal anesteziye bağlı en sık ve en ciddi komplikasyon olan hipotansiyona, profilaktik intravenöz efedrin kullanımı ile intratekal epinefrin ilavesinin katkılarını karşılaştırdık. Gereç ve Yöntem: Toplam 45 hasta dahil edilen bu çalışmada, hastalar rastgele 3 gruba ayrılarak spinal anestezi uygulanarak, bir gruba %2 konsantrasyonda 80 mg prilokain bir gruba profilaktik intravenöz 5 mg efedrinle birlikte %2 konsantrasyonda 80 mg prilokain ve bir gruba da %2 konsantrasyonda 40 mg prilokaine ilave 200 µg epinefrin verildi. Hastalar spinal anestezinin yapılmasından hemen sonra başlayarak, operasyonun sonuna kadar, belirli aralıklarla sensorial blok (bilateral pinpirik), motor blok (Bromage skalası) ve anestezik özellikler kardiyovasküler ve elektrokardiyografik parametreler açısından takip edildiler. Bulgular: Her üç grup içinde de yeterli ve etkili sensorial blok oluşurken, epinefrinli ve efedrinli grupta kardiyovasküler komplikasyon açısından saf prilokain'e göre anlamlı düşüş vardı ve bu durum epinefrinli grupta belirgindir. Epinefrinli grupta hipotansiyon gelişmezken, saf prilokain kullanılan grupta ciddi hipotansiyon ve bradikardi oluşmuştur. Sonuç: Epinefrinin, alt ekstremite kısa ve orta süreli operasyonlarında spinal prilokaine ilavesi anestezik maddenin miktarını yarı doz azaltma avantajı sağlayarak, sistemik toksisite ve kardiyovasküler komplikasyon gelişimini azaltmıştır ve aynı düzeyde etkili ve yeterli anestezi sağlanmıştır.

Abstract

Objective: In this study, we compared the effect of the addition of intrathecal epinephrine and prophylactic intravenous ephedrine on the hypotension, which is the most frequent and serious complication of spinal anesthesia, in the lower extremity surgery. Material and Method: A total of 45 patients were included in this study, and divided randomly into 3 groups. Patients were administered spinal anesthesia. The first group received 80 mg 2% prilocaine, the 2nd group received 2% prilocaine 80 mg in addition to 5 mg intravenous ephedrine and the 3rd group was given 2% 40 mg prilocaine and 200 mg epinephrine. The patients were followed in regular intervals by means of cardiovascular and electrocardiographic parameters, sensorial block bilateral (pin-prick), motor block (Bromage scala) and quality of anesthesia. Results: While there were effective and sufficient sensorial block in all groups, in the pure prilocaine group (group I), cardiovascular complications were found to be significantly higher. In the group with epinephrine, there were no hypotension, where as serious hypotension and bradycardia were observed in the pure prilocaine group. Conclusion: Supplemantation of epinephrine to prilocaine in spinal anesthesia for short and medium duration operations of lower extremity enabled us to lower the amount of the anesthetic medication to the half, thus sustaining the same effective level and sufficient anesthesia. Systemic toxicity and cardiovascular complications were also diminished.
